About the job

Do you want to help shape the future of the local healthcare system?

As a result of the Health Reform, the health and care stations will transfer to regional responsibility and operation as of January 1, 2027. The health and care stations are aimed at citizens who require intensive health professional interventions such as rehabilitation, observation, treatment and care, but who do not require hospital admission.

The task is anchored at North Zealand Hospital in a new organizational unit under the Department of Elderly and Multimorbidity and covers seven geographic locations in North Zealand. In Halsnæs there will be 18 health and care stations, located at Søndergade 81A in Hundested.

Here we will bring together strong health-professional and rehabilitative efforts around citizens with complex needs, and especially for the health and care stations in Halsnæs, we will build a completely new team of physiotherapists and occupational therapists. You will therefore have a unique opportunity to help shape the development of a new regional offer and the building of a strong team-based and interdisciplinary environment with a focus on a good and coherent patient journey.

When the region first takes over responsibility for the upcoming health and care stations on January 1, 2027, you will start your employment in the Department of Elderly and Multimorbidity at Frederikssund Hospital on December 1. There you will receive a thorough introduction to the patient pathways, and you will learn how to use the Health Platform, which is the electronic journal system that will be on the health and care stations from January. From January your permanent workplace will be in Halsnæs.
