Maternity-leave cover as a nurse with the role of cross-sector coordinator sought for the Medical Department for Kidney, Hormones, Elderly and Dietitians

Then the role as a cross-sector coordinator in our clinical inpatient ward may be something for you. Om afsnittet You become part of a clinical inpatient ward at Esbjerg and Grindsted Hospital, where we strive for high professional quality, a good working environment and cohesive patient pathways.

We are an inpatient ward where we prioritize close interprofessional collaboration between nurses, social and health care assistants and doctors, as well as our own physiotherapists and department dietitian.

We have a close collaboration with the municipalities in the hospital's catchment area (Esbjerg, Fanø, Vejen, Varde and Billund), general practice and psychiatry.

Din funktion og organisatoriske placering As a cross-sector coordinator you are employed with managerial reporting to the chief nurse. You are a nurse on our medical inpatient ward for renal, hormonal and geriatric diseases, where you participate in the regular duty rota when you are not performing the coordinator function.

Your core task is to ensure coherence in the patient pathway, particularly in connection with discharge, and to strengthen cross-sector collaboration.

Dine primære opgaver As a cross-sector coordinator you create an overview of upcoming discharges through participation in morning conferences and close dialogue with the nursing staff. You have ongoing contact with home care, hospice, other departments and psychiatry and you are in dialogue with the patient and relatives about complex discharge processes, expectations and needs. You ensure updating of the care pathway plan and/or discharge report, coordinate relevant multidisciplinary actions and ensure that all patients are offered and have documented a needs assessment at discharge.

We expect that you either have, or acquire, in-depth knowledge of SAMBO agreements, municipal and preventive services as well as screening/assignment functions, and you help identify bottlenecks in eligibility and discharge processes and work for improvements. Upon admission you screen for unsettled home conditions, assess needs and initiate actions that create security for the patient and relatives.

You contribute to developing better patient pathways, prevent unnecessary readmissions and take the initiative to take over particularly complicated discharge processes in collaboration with municipal screening/assignment and home care, so the patient experiences a coherent and safe pathway across sectors.

• Sikre oplæring af nye kolleger og opdatering af eksisterende kolleger i SAMBO- og sundhedsaftaler
• Indgå i introduktion af nye medarbejdere i afsnittet
• Bidrage aktivt til at udvikle og videreudvikle funktionen som tværsektoriel koordinator
• Deltage ved løbende samarbejdsmøder – både selvstændigt og sammen med afdelingsledelsen – med bl.a. praktiserende læger, kommuner, interne netværk og andre relevante samarbejdspartnere
• Deltage i relevante kvalitetsudviklingsprojekter
• Udbrede erfaringerne fra dagligdagen til fælles læring for ledere og medarbejdere i afsnittet

Dine kvalifikationer Vi søger en kollega, der

• Er uddannet sygeplejerske
• Gerne har erfaring fra og kendskab til, både primær og sekundær sektor, men det er ikke et krav
• Har overblik, arbejder selvstændigt og kan håndtere mange opgaver samtidigt
• Er fleksibel, har gode samarbejds- og kommunikationsevner, og trives i en funktion med mange kontaktflader og tværfaglige samarbejdspartnere.
• Har lyst og evne til aktivt at medvirke til at udvikle funktionen og det tværsektorielle samarbejde

Vi tilstræber, at kompetencerne i funktionen løbende udvikles, så du opnår et højt fagligt niveau i rollen som tværsektoriel koordinator.

Arbejdstid og rammer

• Stillingen er på 37 timer pr. uge, med en overvægt af dagvagter
• Weekendvagt hver 3. weekend, som udgangspunkt i aften eller nattevagt
• Når funktionen ikke varetages, indgår du i afdelingens almindelige vagtplan

Løn og vilkår efter gældende overenskomst. Regionen er forpligtet til at oplyse dig om startlønnen for stillingen. Du får oplyst grundløn, centralt fastsatte tillæg og pension i overenskomst 32.17.1 . Udover startlønnen, kan lønnen indeholde eventuelt forhandlede tillæg afhængigt af stillingen, dine erfaringer og kvalifikationer.

Ansøgning Ansøgningsfrist er d. 22/11-26, med forventede samtaler d. 25/11-26. Ansættelsen er med forventet start d. 1/1-27, eller efter aftale.

Ansættelsen som tværsektoriel koordinator er et begivenhedsbestemt vikariat, med mulighed for fastansættelse i en basisstilling i afdelingen efterfølgende.
