Nordfyns Municipality seeks a dementia coordinator with strong professional qualifications

Do you have solid knowledge about dementia and strong relational skills? Do you thrive in coordinating citizen pathways and working across disciplines? Are you motivated by making a difference for people with dementia and their relatives? Then you may be our new dementia coordinator.

About the position

As a dementia coordinator in Nordfyns Municipality you will become part of Adult Welfare and one of four dementia coordinators in total, focusing on professional coordination, advice, sparring, and development in the dementia field. We support both people with dementia, relatives, employees in home care and on care homes, as well as civil society and other partners.

This is a newly created position to increase the overall capacity in the team, and we look forward to welcoming a new colleague! The three existing dementia coordinators are an occupational therapist, a nurse, and a physiotherapist, and we are looking for a colleague who can supplement and strengthen the overall professional breadth. If you have an interest in or competencies within a particular area, we would like to hear more, as there is room in the group to specialize and share the workload.

In addition to the other dementia coordinators, your closest colleagues will be two home visitors, a field-based activity worker, and the staff at the dementia activity offering Højskolen 5-kanten. A professional leader is attached to the entire group.

We work from a person-centered perspective with respect for each individual's self-determination and with a strong cross-disciplinary approach that supports the individual's possibility to live an independent and meaningful life. Even small efforts can make a big difference in daily life, and we work to ensure that people with dementia and their relatives experience coherence, security, and support to live the life that makes sense to them.

Your main tasks

• Advice and guidance to citizens with dementia and their relatives, including home visits and telephone follow-ups
• Coordination and follow-up of pathways in collaboration with relatives, dementia clinic, home care, care homes, activity offerings, home visitors, general practitioner, etc.
• Professional sparring, cross-disciplinary meetings, and knowledge sharing with colleagues and partners
• Facilitation of relative groups and/or relative education
• Contribute to quality assurance and development of dementia-related initiatives across sectors and civil society
• Communication and implementation of new knowledge and applicable guidelines

An example of a workday

You arrive at the office in Otterup, turn on the coffee machine and participate in a short online check-in meeting, where daily discharges are coordinated and there is brief cross-disciplinary sparring across the municipality. You answer emails, discuss a case with a colleague, and answer the phone when a relative calls. In Nexus you can follow messages and tasks regarding the citizens you are attached to. You contact two families and arrange follow-up visits next week, before you drive to an already planned visit.

Your one colleague has started from home, but you meet at the office during the day and talk about both work and a family trip this weekend. After documentation and telephone follow-ups, the day ends with a cross-disciplinary citizen conference in Søndersø.

• have a medium-length health-related education, preferably with continuing education in dementia
• have experience with the dementia field and with working with people with dementia and relatives
• are empathetic, professional, and clear in your communication

• are willing to teach colleagues, partners, students, relatives, and volunteers
• are familiar with documentation in Nexus or a similar system
• have knowledge of the use of power and rules regarding guardianship and self-determination. It is an advantage

The practical details

• The position is between 30-37 hours, and your base will be at Bryggerivej 20, 5450 Otterup.
• As a team we cover the entire municipality, so home visits, meetings, and similar activities take place across the municipality.
• As a self-managed employee there is the possibility of working from home.
• Salary and terms of employment in accordance with the applicable collective agreement. Expected salary range will be announced at the first interview. The final salary is determined based on your qualifications and previous work experience and after concrete negotiations with the relevant trade union.
• Criminal record check must be obtained before employment can take place.
• You have a driver's license and if you use your own car you will be paid according to SKAT rates.
