About the job

Would you like to use your nursing professionalism in a daily life with independence, a strong collegial community and close interdisciplinary collaboration?

We are looking for an engaged nurse for our outreach nursing and therapy team in District Midt. Here you will be part of a work community where we help each other and collaborate to create cohesive and safe pathways for citizens.

A central role in the citizens' pathways As a nurse in a miniteam you will play an important role in both complex and acute citizen pathways. You will be the coordinator and help ensure that the interventions hang together across professional groups and partners. The position requires that you thrive with clinical decision-making, can maintain an overview and translate your professional knowledge into action – even when everyday life changes.

Your tasks will include

  • Planned and acute nursing tasks in the citizens' homes
  • Clinical assessments and decisions
  • Triage in close collaboration with your miniteam
  • Documentation, follow-up and evaluation
  • Coordination of complex and interdisciplinary citizen pathways
  • Rehabilitative interventions that support the citizen's resources and independence
  • Contribution to professional development, sparring and well-being in the team

As an outreach nurse you will encounter situations where the solution is not always found in a manual. Therefore it is important that you can think creatively, act professionally and find solutions together with the citizen, relatives and colleagues.

We are looking for you

  • Are a licensed nurse
  • Have an interest in, and preferably experience with, working with elderly citizens
  • Work systematically, responsibly and independently
  • Thrive in interdisciplinary collaboration and shared responsibility
  • Can create an overview and prioritise in a varied and unpredictable daily life
  • See opportunities rather than limitations
  • Meet the citizens with professionalism, respect and care
  • Contribute actively to a good work environment

With us you get

  • A permanent position of 32-37 hours per week
  • Primarily day shifts from 7:00 to 15:00
  • Weekend shift every 4 weeks
  • Flexible scheduling
  • A strong professional and social work community
  • Engaged and helpful colleagues
  • Ongoing professional sparring and good development opportunities
  • A workplace characterized by trust, openness and good humor
  • A meaningful and varied daily life where two days are rarely the same

We have room for both single-discipline specialization and interdisciplinary development. At the same time we value diversity and believe that the best solutions are created when multiple professions work closely together.

Become an important professional voice As a nurse with us you will have the opportunity to make a tangible difference for the citizens and actively put your professional knowledge into play. So if you would like an exciting, challenging job in a fantastic staff group, we would love to hear from you

Start: January 1, 2027 or by agreement.

Application deadline: November 20, 2026.

If you would like to know more about the position, you are welcome to contact Department Manager Maria Jelø at phone 30 84 13 86.

Salary and terms of employment follow the applicable collective agreement. A criminal record check and references will be obtained before employment.

We look forward to hearing from you.
