At Svane Pleje we work in a team-based structure. You will be attached to one or more teams consisting of a planner, SOSU assistants, SOSU helpers and unskilled workers. You will become part of an engaged therapy team across all our 14 departments, where there is an opportunity for sparring.

About the position

The tasks as a physiotherapist span broadly within prevention, maintenance, rehabilitation and recovery. You will function as the primary case manager for citizens referred under the Elderly Act, including clarification of help needs and assessment of rehabilitation potential as well as planning the citizen's progression thereafter. There will also be tasks with rehabilitation of citizens in the department's training room.

You will also have the task of supporting that all SOSU assistants, SOSU helpers and unskilled workers work rehabilitatively, which we imagine happens through daily sparring with colleagues, participation in team meetings and interdisciplinary meetings, teaching and supervision. It is therefore a prerequisite that you are curious and eager to take on an advisory role regarding your colleagues' professional development.

You will have the opportunity to generally contribute to quality and development tasks, for example we will implement welfare technology solutions and strengthen collaboration with voluntarism and civil society.

A typical day starts out with the citizens, where you, for example, start a rehabilitation programme, train the citizen to become self-reliant, perform a workstation assessment or support your colleagues in daily rehabilitation. Later you may have administrative tasks in the office, where you order assistive devices, perform documentation practices and the day may end with an interdisciplinary meeting.

Your tasks

• As the primary case manager you shall assess the citizen's rehabilitation and recovery potential and initiate the right help or training and ensure ongoing follow-up on the progression
• Involve colleagues in the citizen's plan, so that everyone works with the citizens' goals and based on the rehabilitative way of thinking
• Carry out rehabilitation individually or as group training
• Ensure professional reflection, facilitate interdisciplinary meetings and team meetings
• Support interdisciplinary collaboration with municipal partners, including authorities
• Focus on voluntariness and the civil society's importance for the citizen's life
• Contribute to training of new employees and support task distribution according to competencies

Application and employment conditions

This is a permanent position of 25-30 hours, where you can help set the weekly working hours at the time of hire. Working hours are typically planned within the period 07:00–16:00, but afternoon and/or evening work may occur.
