Would you like to help develop a new ambulant treatment offering in Psychiatry Randers?

As of 1/10-26 we have established a new Subacute Team in the Psychiatric Clinic Randers. The team offers an intensive, time-limited ambulant intervention for adults with acute worsening of mental illness with the aim of preventing hospitalizations, supporting earlier discharge and creating coherent patient pathways.

About the position

The position is a full-time maternity leave cover. You will become the health care professional contact person for your own patient journeys and work both from the clinic in Randers and in the patients' homes or local environment. The work takes place in close collaboration with patients, relatives, municipalities, general practice and other partners.

An important part of the role is also to conduct subacute assessment interviews . The purpose is to quickly identify a possible psychiatric issue in patients who do not require acute admission but who cannot wait for ordinary diagnostic evaluation. The interviews aim to triage the patient to the correct intervention and create alternatives to hospitalization. A subacute interview can, among other things, lead to discharge to the patient's GP, referral to the Subacute Team, ambulant treatment or evaluation, municipal offers or admission if deemed necessary.

Your tasks

• Manage your own patient pathways as the health care professional contact person.
• Conduct ambulant consultations, home visits and telephone follow-ups.
• Hold subacute assessment interviews and contribute to rapid visitation and triage.
• Develop and follow up on treatment plans.
• Observe disease progression, effect and side effects of treatment.
• Initiate and evaluate relevant nursing interventions.
• Coordinate the effort with patients, relatives and partners.

We are looking for someone who

• is an experienced nurse or an experienced social- and health care assistant
• has experience from psychiatry
• works independently and takes responsibility
• has good clinical observation and assessment skills
• can create safe relationships and work recovery-oriented
• thrives with both acute and planned tasks
• is accustomed to documenting in EPJ
• wants to contribute to the development of a new offering.

A driving license is a prerequisite, as part of the work takes place in the patients' homes.

Employment

The position is a full-time job with working hours on weekdays between 08:00 and 16:00.

Salary and terms follow the applicable collective agreement.
