About the job

We are seeking regional visitators for a new function at Bispebjerg and Frederiksberg Hospital

Do you have strong clinical experience, a good sense of the overall patient pathway, and an interest in working across the hospital and municipality? Then perhaps you are the one we are looking for.

From January 1, 2027, the regions will have a new task with visitation to health and care placements. At Bispebjerg and Frederiksberg Hospital we are therefore establishing a new regional visitation function, and we are seeking several regional visitators to help build and develop the function.

A common function across the hospital

As a regional visitator you will be part of a common function for the entire hospital, but you will be organizationally employed in the Geriatric and Palliative Department. Here you will have your organizational affiliation and daily management, while your task is directed at patients and departments across the whole Bispebjerg and Frederiksberg Hospital.

In the municipalities of Copenhagen and Frederiksberg there are a total of 239 health and care placements spread across several sites, to which the regional visitation function will conduct visits.

You will assess patients who no longer need to be hospitalized but who are not yet ready to go directly home, and decide whether a health and care placement is the right offer.

You will, among other things

  • assess patients for health and care placements.
  • assess the patient's overall medical, functional and rehabilitative needs.
  • match the patient with the relevant offer and have an overview of capacity.
  • coordinate the transition from hospital to health and care placement.
  • collaborate with hospital departments and the municipalities.
  • follow up on patient pathways where changes or additional coordination are needed.

You will thus become an important link between the hospital, the municipalities and health and care placements.

You must be able to see both the patient and the overall flow

The task requires you to balance the consideration for the individual patient with an eye for capacity and flow. You must be able to work closely with clinical staff, create an overview of complex pathways, and at the same time make an independent visitation-professional assessment.

We will develop the function together

The function is new, and you will have the opportunity to help shape workflows, collaborations and the professional practice across the hospital's departments and in cooperation with the municipalities.

We do not expect you to have all the answers. We expect that you are willing to take responsibility, ask questions and help to find solutions.

Who are you?

You have a health-professional education as a nurse, physiotherapist or occupational therapist and relevant experience from the health service.

This could include experience from hospital, municipality, rehabilitation, geriatrics, medicine, acute care, or other areas with complex patient and citizen pathways.

It is an advantage if you have experience with discharge, coordination, cross-sector pathways or municipal offers. Experience with visitation is also relevant, but not required.

We particularly emphasize that you

  • have a solid clinical perspective and be able to assess the needs of complex patients.
  • can create an overview and make decisions on a professional basis.
  • work independently and in a structured way.
  • communicate clearly and have good relational skills.
  • can see both the patient's needs and the overall pathway.
  • have an interest in flow, capacity and organization.
  • thrive in a cross-department role across the hospital.

We offer

  • a newly created position, where you will have the opportunity to shape the function from the start.
  • an interdisciplinary team with different health-professional perspectives.
  • close collaboration with the hospital's departments and municipal partners.
  • a workday with variation and independent responsibility.
  • opportunities for professional development and to influence the further development of the function.

Practical

The position is as a rule 37 hours per week. For special requests, other arrangements can be agreed.

Working hours will primarily be on weekdays during daytime. The function must, however, be available every day of the year, and there will therefore be some weekend and holiday duties. In the startup phase this will involve being present at the hospital, but in the long term it may be possible for weekend and holiday shifts to be covered as on-call duty from home.

Salary and employment terms follow the applicable collective agreement.

Start date: December 1, 2026 or as soon as possible thereafter.

Do you want to be part of it from the start?

If you want to use your health-professional experience in a new way and help create smooth transitions between the hospital and the municipality, we would love to hear from you.

If you want to know more about the position or the new function, you are very welcome to contact us.

Application deadline: Sunday October 18

Interviews: Wednesday October 21 and Thursday October 22
