At the Health and Care Pladserne in Ø. Tørslev we are seeking dedicated social and health care assistants who want to become part of an exciting startup and help create the best pathways for citizens in need of temporary health and care stays.

As a new employee you will have the opportunity to come aboard before the task is taken over from the municipalities. That means you will receive a thorough introduction to the target group, workflows and the systems you will work in, so you are well prepared for the task from day one.

As a social and health care assistant you will therefore be a central part of a strong multidisciplinary collaboration with nurses, therapists, social and health care aides and other partners.

Your tasks

You will have a diverse daily routine, where you among other things shall

• Carry out care and nursing tasks based on the individual citizen's needs and goals.
• Actively contribute to rehabilitative efforts and support the citizen's functional abilities and independence.
• Perform training-related tasks in collaboration with therapists and other professional groups.
• Perform nursing-related tasks within the social- and health care assistant's competence area.
• Observe, assess and respond to changes in citizens' health status.
• Document and follow up on interventions and goals.
• Contribute to coordination and coherence in citizen care pathways.

Practical information

The position is 37 hours, however there will be an opportunity to agree on taking accrued allowances as days off, which may result in a reduction in actual attendance time; the position involves mixed shifts and is desired to be filled by 1.1.2027 or by agreement.

Weekend work is expected every third weekend.

Salary and terms of employment follow the applicable collective agreement and the principles of local wage formation.
