Social- and health-care assistant for Health and Care Placements Thyregod, Near Health, Lillebaelt Hospital

Would you like to be part of a very exciting task related to the implementation and establishment of the new Health and Care Hubs in Thyregod?

You will work in new and updated physical frameworks, and you will be an important player in building our new department.

We are looking for a licensed social and health care assistant who is passionate about complex citizen pathways, rehabilitation, and interdisciplinary collaboration in the local health care system.

The position is generally 37 hours/week; if you prefer a lower hourly rate, there is also an option for that.

You will be included in a duty roster with weekend shifts. You can be offered a daytime position, which may also include evening or night shifts, or a position with a fixed shift type.

Health and Care Hubs Thyregod The staff group consists of social and health care assistants, nurses, occupational therapists, physiotherapists, house assistants, service assistants, and medical coverage from hospital and general practice.

Our core task is to receive citizens with complex needs for specialized nursing, rehabilitation, and palliation of a nature that cannot be solved in their own home, and a main task is to build a bridge between the sectors.

Citizens shall be rehabilitated through close interdisciplinary collaboration, so that their strengths in relation to resources and health competencies are enhanced, thereby regaining independence and a higher quality of life.

There will be close cross-sector collaboration with emergency nursing in Nær Sundhed, general practitioners, responsible treatment wards, training, and municipal home help and nursing. The goal is for multi-ill citizens to safely return home and for readmissions to be prevented.

About the position As a social and health care assistant at the Health and Care Hubs in Nær Sundhed, the core task is to ensure that citizens with complex health needs receive a coherent, professionally qualified and secure intervention in the follow-up after discharge or as an alternative to hospitalization.

Our task is to strengthen functional capacity and well-being for the individual citizen.

You will follow a structured and well-planned introductory program. You will also have good opportunities to develop your competencies.

Who are you? You are a social- and health care assistant and enjoy playing a central role in the daily work around the citizen, including:

• Observation, care and treatment of citizens with various and complex health needs.
• Rehabilitation based on the citizen's own goals and resources.
• Medication management and systematic follow-up on the citizen's condition.
• Close interdisciplinary collaboration with nurses and therapists.
• Dialogue and collaboration with the citizen and relatives.
• Documentation and knowledge sharing across professional groups.
• Planning and coordination of coherent processes and transitions to home.
• Participation in work on quality, patient safety, and development of our practice.

Salary and employment terms Salary and terms of employment for the position follow the Social and Health Care Personnel – Collective Agreement 32.03.1 - salary as of 1.4.2026. In addition to the starting salary, the pay may include negotiated allowances depending on the position, your experience and qualifications.
