Social- and Health Care Assistant for Home Care District North – Day Shift

We are in the midst of an exciting development with the Elderly Reform, where proximity, quality and the citizen's self-determination are in focus. We therefore seek a social- and health care assistant who wants to actively contribute to the development and make a difference for the citizens.

The position is 37 hours per week in day shifts with weekend shifts every 2nd or 3rd weekend by agreement.

Your tasks

You will become an important part of the citizens' daily life, where you use your professional skills to ensure quality and coherence in care.

Your tasks include, among other things, to

• take care of care, nursing and health-related tasks based on the citizen's needs
• observe changes in citizens' condition and work with early detection
• support citizens in maintaining their functions and manage as much self-sufficiency as possible
• contribute to coordination and interdisciplinary collaboration
• document systematically in CURA.

We also work with the development of dispensing clinics and new workflows that will strengthen the quality and safety in medication management.

We are looking for someone who

• is trained as a social- and health care assistant and has a strong professional approach
• can work independently and at the same time thrive in close collaboration with colleagues
• has an eye for early detection, prevention and rehabilitation
• takes responsibility and contributes to good citizen pathways
• is curious and dares to challenge habits when something could be done better
• contributes to good communication and a positive working environment
• has a driver's license.
