Social- and Health Care Assistant for Home Care West

We are seeking a new colleague for an evening shift position starting 1 December 2026 or as agreed.

We work in fixed interdisciplinary teams, where continuity, presence and high quality in citizens' care pathways are at the center.

We are based at the Activity Center on Brorsonsvej and meet in person as well as on screen with nurses and therapists – making sparring and collaboration a completely natural part of our daily routine.

About the Position

As a colleague in the team, you will help ensure continuity, presence and high quality in the citizen’s daily life.

The position can be 37 hours per week, with work every second weekend. We are flexible if you want a lower or different number of hours. As a starting point, the team works in a 5:2 arrangement.

Your tasks

• Ensure quality and continuity in the citizens' care and treatment
• Perform tasks according to competence and delegation
• Coordinate and actively participate in our interdisciplinary meetings – trained in CuraPlan
• Create an overview of citizen care pathways and contribute to planning of the day’s tasks
• Engage in close collaboration with colleagues, nurses and therapists
• Contribute to professional sparring and development in everyday life

Your profile

• You are trained as a social and health care helper
• You have a solid professional foundation and can reflect on your own practice
• You are willing to take responsibility and work independently
• You work from a holistic perspective
• You thrive in close collaboration with colleagues and the nursing staff
• You contribute to a good working atmosphere
