Social and health care assistant for a mobile outreach team at Kildemarkscentret, District Southeast

With us: We are social and health helpers, social and health assistants, occupational therapist, nurses, physiotherapist, apprentices, students, operation support staff and team leader. We work in day and evening shifts.

We are a team with a strong focus on professional sparring, knowledge sharing and development. We work purposefully to create the best possible balance between work and private life.

Over the last few years, we have worked purposefully with daily triage, and the conduct of these meetings is now handled by all professional groups.

We work diligently with planning of the allocated tasks, and gradually we have become familiar, also with that task.

We hold weekly Citizen Conferences with participation of the visitator, to ensure near visitation and close follow-up on citizens' current and individual needs.

We have individually tailored introduction programs, and you have the opportunity to put many competences into play, both somatic, psychosocial and collegial.

• You are trained as a social- and health helper
• You are open and approachable
• You apply your professional knowledge in interaction with the citizen and relatives as well as in the collegial community
• You believe that competence development is a natural part of the profession
• You work professionally and focus on solutions and possibilities

You want a close-knit job where you give a lot of yourself, but also get a lot in return, and see it as natural to collaborate across with citizen, colleagues, relatives and other partners. During the day, the citizens are visited by bicycle.

Salary - and employment conditions are in accordance with the applicable collective agreement in place between KL and FOA.
