Social worker or social mediator for the Rehabilitation Department, Psychiatry South

At the Rehabilitation Department "The Four Corners" we are looking for a social worker or social facilitator who wants to combine social work with daily environmental therapy and a contact person role.

With us, your social work knowledge becomes an important part of the patients' overall treatment and rehabilitation journey.

A ward with a special task and a strong multidisciplinary community

The Four Corners offers longer, holistic treatment and rehabilitation pathways of up to 9 months for people with severe mental illness and concomitant complex substance use issues. We work recovery-oriented and take as starting point the patients' own goals, wishes and possibilities to create a more stable and meaningful life.

The patient group is complex and requires high professionalism, patience and relational presence. We are looking for a colleague who meets people with curiosity, respect and belief in development, and who can help carry the vicarious hope when the challenges are greatest.

You will become part of an interdisciplinary team including, among others, nurses, pedagogues, social and health care assistants, occupational and physiotherapists, peer workers, doctors, psychologists and substance abuse therapists. We emphasize that you actively contribute with your social work knowledge and your perspectives in the joint work around the patients.

Your tasks as a social worker or social facilitator

You will have a central role in working with the patients' social conditions and in creating coherence between their treatment and life outside the ward. At the same time you will be a natural part of the daily environmental therapeutic work, where you help create structure, relationships and meaningful activities in the patients' daily lives.

You will also carry a contact person role and support the patients in their individual treatment and rehabilitation process. The position therefore combines your social work specialist knowledge with direct, relational work close to the patients.

Your tasks will include, among others

• To actively participate in the daily environmental therapeutic work and support a safe, structured and meaningful everyday life for the patients.
• To carry a contact person role with a focus on relationship, continuity and the patient's individual goals.
• To identify and work with the patients' social challenges, resources and possibilities.
• To provide social work counseling and guidance on, among other things, finances, subsistence, housing conditions and social support opportunities.
• To collaborate with municipal authorities, transitional housing and other relevant partners on the patients' overall pathway.
• To support good transitions between hospitalization, municipal offers and the patients' further life.
• To contribute with social work assessments and knowledge of legislation in the multidisciplinary treatment planning.
• To support the patients in understanding their options and rights and actively participate in their own process.
• To contribute to an open and reflective work environment with professional sparring and shared learning.

We work from a fixed treatment structure, but adapt the effort to each patient’s needs and goals.

Who are you?

You are a trained social worker or social facilitator and preferably have experience from psychiatry, social psychiatry, the substance area or municipal authority treatment.

• Have a solid social work understanding and a desire to actively bring your knowledge into play.
• Thrive with direct patient contact and want to engage in both environmental therapeutic work and contact person responsibilities.
• Can see the connection between social, psychiatric and substance-related problems.
• Are good at building relationships and collaborating with patients, relatives and external collaborators.
• Can work independently and in a structured way while also contributing professionally in a close multidisciplinary collaboration.
• Are keen on longer rehabilitation pathways where small advances can have great significance.

Working hours and shift structure

The position is 37 hours per week with work every fourth weekend. Evening shifts may occur as part of the shift planning.

Starting salary for employees under collective agreement

The region is obliged to inform you of the starting salary for the position. You will be informed of base pay, centrally fixed allowances and pension in the relevant collective agreement. In addition to the starting salary, the pay may include any negotiated allowances depending on the position, your experience and qualifications.
