About the job

Do you want to help make a difference for people in vulnerable positions and help create coherence between patient, hospital, municipality and other offers?

To the Social Team at Holbæk Hospital we are looking for an engaged and relationship-oriented nurse with an interest in social nursing and work with people in complex and vulnerable life situations.

As a social nurse you will be part of a multidisciplinary team where, with your nursing expertise and human insight, you help build bridges between patient and system and create trust, security and access to relevant treatment.

The Social Team works across Holbæk Hospital and consists of social nurses and a social worker. We work closely with the hospital departments and with municipalities and other actors around people in vulnerable positions.

As part of the position you may also have tasks in Flex Clinic, which is a new offer for socially vulnerable citizens. The Social Team and Flex Clinic work closely together and have, among other things, a shared morning conference every day at 8:15, where we coordinate, advise and together create an overview of the patients and issues that cross.

The Social Team and Flex Clinic

The Social Team's task is to support good and coherent patient pathways for socially vulnerable patients at Holbæk Hospital. As a social nurse you will have a bridging and coordinating function between the patient, hospital staff, the municipality and other relevant partners.

Flex Clinic is established as a special general medical offer for socially vulnerable citizens who for various reasons have difficulty using the existing services in the health care system. This can include people with alcohol or drug use, homelessness, mental illness, dual diagnoses or multimorbidity.

The clinic aims to help people in complex life situations gain access to the necessary medical care and to more coherent pathways across general practice, hospital and municipality.

The Social Team and Flex Clinic are organizationally anchored in the Department of Work and Social Medicine and work closely together. We see it as a strength that staff can work across because many of our patients move between different parts of the health care system. Tasks are therefore planned with the starting point of the patients' needs and the professional competencies of the staff.

The Department of Work and Social Medicine comprises almost 60 staff in total, including specialists with backgrounds in social medicine, occupational medicine, general medicine and psychiatry, training doctors, medical secretaries, social nurses, social workers, psychologists, senior researchers and PhD students.

Your role and your tasks

Your primary function will be as social nurse in the Social Team , where you, together with your colleagues, work across Holbæk Hospital.

An important part of the work is to create security and overview for socially vulnerable patients during hospitalisations and outpatient courses. You will among other things help strengthen the dialogue between patient and health professionals and contribute to the patient's needs and life situation being considered in the treatment.

Your tasks can include, among others, to

  • support socially vulnerable patients during hospitalisation and in outpatient courses
  • establish contact and build trustful relationships with patients who may have difficulty navigating the health service
  • advise and guide in relation to, among other things, substance use, withdrawal, mental illness and social vulnerability
  • contribute to creating coherence and coordination between patient, hospital, municipality, general practice and other partners
  • support hospital staff in encounters with patients with complex social and health-related problems
  • contribute to finding pragmatic and individually tailored solutions together with the patient and other involved parties.

Additionally there may be tasks in Flex Clinic

As the Social Team and Flex Clinic work closely together, some of your tasks may also be in Flex Clinic. Periodically you may therefore have several days in Flex Clinic. This can include:

  • conversations and relationship work with the clinic's patients
  • blood tests, ECG, wound care and other relevant nursing tasks in close cooperation with the clinic's doctor
  • answering the phone in Flex Clinic
  • contact person function for patients where continuity and a trusting relationship are important
  • medication management in collaboration with the doctor and depending on your competences
  • bridging between Flex Clinic, the hospital and other services
  • potentially a mobile function where patients meet in their local environment.

The possibility to follow the patients and collaborate across boundaries is important to us. This means that the work does not always follow organizational boundaries, but as far as possible is based on what makes sense for the patient.

We do not expect you to know everything from the start

The most important is that you are curious, solution-oriented and want to develop yourself both professionally and personally.

You will become part of a work environment with a focus on professional sparring, interdisciplinary collaboration and well-being. Ongoing supervision and education are offered as part of professional development.

Flex Clinic is still a service under development, and you will therefore also have the opportunity to help shape the collaboration between the Social Team and the clinic and the development of our overall effort for socially vulnerable patients.

Who are you?

We are looking for a nurse who is professionally skilled, engaged and interested in social nursing and working with people in vulnerable positions.

You meet the patients in a respectful and accepting manner and are good at building trust. You can see the person behind the problems and support the patient's dignity, quality of life and opportunities for mastery in their own life.

We also place emphasis on that you

  • have experience with clinical nursing from areas such as somatic care, psychiatry, general practice, municipality, substance area or vulnerable populations.
  • understand the organization and framework of the health care system
  • can work independently, but also thrive in close interdisciplinary collaboration
  • are good at building bridges and coordinating
  • can be creative and pragmatic when the standard solution does not fit the patient's situation
  • can identify and understand the patient's needs and health competencies and communicate this to other professionals
  • preferably have knowledge about drugs, withdrawal, homelessness, mental illness, vulnerability and marginalization.

And we definitely see it as an plus if you also have a good sense of humor and contribute positively to the community.

The position is initially 37 hours per week, but part-time is possible. Salary and employment terms are according to applicable collective agreement.
