About the job

Do you want to help create more health equity for some of the municipality's vulnerable citizens?

We are seeking a motivated and professionally strong public health nurse who is passionate about working holistically with citizens who often fall between the systems and have complex social, psychiatric, and somatic challenges.

As a public health nurse you will be part of a specialized function in Health and Care that supports employees and citizens across Aarhus Municipality. You will play a central role in strengthening the coherence of citizen paths and creating access to health services for citizens who often find it difficult to accept help.

About the position

You will be organizationally attached to the Health Unit Brabrand, which serves as your fixed base. The public health nursing function works across home care and health units and covers the entire Aarhus Municipality. A substantial part of the work therefore takes place at citizens' homes and with partners at different locations in the municipality. You should therefore thrive in a workday with a good amount of travel and where tasks are carried out close to the citizens’ daily lives.

The work is anchored in the citizens' overall life situation and focuses on creating health equity through relationships, coordination, and close collaboration with both the citizen and relevant partners.

The public health nursing is a time-limited intervention, where you support citizen pathways without taking over the overall care responsibility.

Your tasks

As a public health nurse you will among other things

  • support known, non-acute citizen pathways with a focus on health promotion and prevention
  • coordinate and support complex citizen pathways with both social, psychiatric, and somatic issues
  • build relationships with citizens where the relationship is a prerequisite for working with health and well-being
  • conduct nursing assessments and investigations focusing on, among other things, medical history, resources and need for support
  • collaborate closely with municipal, cross-magistrate, cross-sectoral and local partners
  • contribute with professional sparring to colleagues regarding citizens in the target group
  • support learning in practice through accompanying guidance and sparring with staff across professional groups

Target group

You will work with socially vulnerable citizens who often live with several simultaneous challenges. This may include citizens who:

  • have difficulty accepting help and treatment
  • have untreated or inadequately treated illnesses
  • live with both somatic and psychiatric issues
  • have substance abuse problems
  • have many hospitalizations or repeatedly refuse necessary treatment

Thus, these are citizens who often need a hand-held, flexible and persistent effort to obtain the help they need.

We are looking for someone who

  • is a licensed nurse
  • has solid clinical experience, preferably from both the somatic and psychiatric fields
  • has experience with or interest in working with socially vulnerable citizens
  • works relationally and sees the person before the diagnosis
  • can navigate safely in complex citizen pathways and many collaboration interfaces
  • is curious, solution-oriented and dare to think new when the common interventions do not suffice
  • has strong pedagogical skills as well as good collaboration and communication abilities
  • can translate nursing knowledge into concrete actions together with citizens, colleagues and partners
  • thrives with a high degree of independence, can plan and prioritize own efforts and works persistently to create progress in complex citizen pathways
  • has a driving licence and a car

Why you should choose us

With us you get

  • a meaningful job with great impact on the citizens' lives and health
  • a specialized function with high professionalism and a lot of autonomy
  • possibility to work across professional areas and sectors
  • dedicated colleagues and strong collaborative relationships
  • professional sparring, reflection and skills development

Salary and terms of employment

The position is 37 hours per week with commencement on January 1.

Salary and terms follow the applicable collective agreement. Final salary is determined after negotiation with the negotiating organization.

The position requires a driving licence, and you must provide your own car. You will receive a travel allowance.

The position is self-scheduling, generally day shifts, but evening and weekend work may occur.
