As our good colleague is going to study to become a nurse, we are therefore looking for a new good colleague to join a strong interdisciplinary community, where you with your professionalism help contribute to holistic care, interdisciplinary collaboration and citizen involvement. We work in fixed interdisciplinary teams, where continuity, presence and high quality in citizen outcomes are central.

The team operates from Munkebo on Farimagsvej 52 and primarily visits citizens who live in central Næstved or in Sct. Jørgens Park and surrounding areas.

The team is interdisciplinary and we work with the improvement model, hold daily triage meetings, where you as a social & health care assistant will also take a central role and are expected to apply your professional knowledge.

We work from the elderly care perspective with a focus on holistic care, staff continuity and the citizen’s self-determination. Rehabilitation and prevention, early detection of dementia, acute nursing and palliation are our professional foundation. We work citizen-centred and are willing to go the extra mile to ensure the best possible effort for the citizens.

About the position

As a colleague in the team, you will be part of ensuring continuity, presence and high quality in the citizen’s daily life.

There is a permanent position of 37 hours per week in mixed shifts, naturally with the understanding that it must work for everyone. The hourly rate can be negotiated. We work every 2nd weekend.

Your tasks

Ensure quality and continuity in citizens’ care and treatment

Perform tasks according to competence and delegation

Coordinate and actively participate in our interdisciplinary meetings

•

You will receive solid training in Nexus.

•

Create an overview of citizen pathways and contribute to planning today’s tasks

•

Collaborate closely with colleagues, nurses and therapists

•

Contribute to professional sparring and development in everyday life

You are educated as a social- and health care assistant

•

You are professionally grounded and can reflect on your own practice

•

You are willing to take responsibility and work independently

•

You have a good clinical eye and work from a holistic perspective

•

You thrive in a coordinating role and in close collaboration with nursing.
