About the job

Healthcare professional for the allocation unit for health and care placements

Do you want to help build a new function that will ensure professionally well-founded allocation of health and care placements and good transitions for citizens? Are you motivated by health professional assessments, cross-sector collaboration and the opportunity to shape the development of the future close-to-community health service? Then you have the opportunity to become a central part of the allocation unit in the Center for Citizen-Near Health at Amager and Hvidovre Hospital.

The health reform has triggered major changes in the health system, and as of January 1, 2027 Amager and Hvidovre Hospital will take over the responsibility for running 138 health and care placements from nine municipalities in the Health Council Amager and the Vestegnen. In this context, the hospital will also take over employees and management responsibility across several sites.

We are therefore seeking an experienced health professional for the allocation unit for health and care placements in the Center for Citizen-Near Health. You will help ensure that the right citizens are offered a health and care placement, that there are good and coherent citizen pathways, and that citizens, relatives and partners experience a health service that works across sectors.

We offer an exciting, newly established position with the opportunity to influence development

As a staff member in the allocation unit, you will play a central role in the assessment of referrals and the allocation of health and care placements. You will work closely with the hospital departments, municipalities and other referring partners to ensure good transitions and appropriate citizen pathways.

The allocation unit will initially consist of two health professionals and a secretary, who support the allocation and completion of citizen pathways across the 138 health and care placements in Health Council Amager and Vestegnen.

You will also work closely with the seven chief nurses on the five sites where the health and care placements are located, as well as with the clinical nurse specialist. Together you will contribute to developing good workflows and ensuring uniform services that support citizens' needs and create coherence across sites and sectors.

A significant part of the task will be to assess and allocate placements and to support the referring partners with the necessary knowledge about referral to and allocation of health and care placements. You will also contribute to strengthening the quality of referrals and ensuring that there are clear targets and plans for citizens' pathways, and that relevant partners are involved at the right times.

You will help create trust, engagement and followership, so that partner organisations experience coherence, a strong professional community and good opportunities to collaborate on citizens’ pathways.

This gives you the opportunity to influence the development of the close-to-community health service and strengthen collaboration between hospital, municipalities and general practice for the benefit of citizens in Health Council Amager and Vestegnen.

Your main tasks

  • Assess referrals to health and care placements based on applicable criteria and the citizen’s overall situation.
  • Make decisions on allocation within applicable target group descriptions, criteria and professional frameworks.
  • Engage in dialogue with referring actors and municipalities when additional information or clarification is needed.
  • Contribute to good transitions from hospital or the citizen’s own home to a health and care placement.
  • Help ensure an overview of and appropriate use of capacity across the 138 placements.
  • Support the quality of the referral process and contribute to consistent workflows across sectors and sites.
  • Contribute to the implementation of the applicable quality standards for health and care placements in collaboration with the clinical nurse specialist, chief nurses and relevant partners.
  • Strengthen the interdisciplinary and cross-sector collaboration with internal and external partners.

Your profile

We are looking for an experienced health professional who is motivated to create good citizen pathways through professional assessments, collaboration and development.

You have a curious and investigative approach to complex pathways and thrive in an organization where development, operations and collaboration go hand in hand. You see opportunities in changes and want to play a central role in building a new function.

You succeed in translating decisions into practice and creating followership, even when circumstances change. At the same time you are focused on creating security and good transitions for citizens and on developing collaboration across sectors.

We place emphasis on

  • Having experience with sector transitions between hospital, municipality and possibly general practice, e.g. from a discharge function at a hospital or as an employee with referral tasks in a municipality.
  • Preferably having experience with health professional tasks on municipal temporary placements.
  • Having strong health professional competencies and being able to assess whether the citizen’s needs fall within the criteria for staying at a health and care placement.
  • Working in a structured manner and being able to make well-founded professional assessments.
  • Having strong collaboration skills and being able to create solutions across sectors for the benefit of the citizen.
  • Having a mindset of an appreciative collaborative culture that promotes engagement and a good working environment.
  • Willing to help build and develop a new function.
  • Able to create followership and engagement around complex citizen pathways.
  • Communicating clearly and being able to build trusting relationships with citizens, relatives and partners.

Center for Citizen-Near Health

The Center for Citizen-Near Health is a newly established unit at Amager and Hvidovre Hospital. As of January 1, 2027, the center will take over the operation of 138 health and care placements distributed across five sites, serving citizens from nine municipalities in Health Council Amager and Vestegnen. In addition, responsibility for emergency nursing in the health council will also transfer to the center.

The health and care placements are aimed at citizens who need a health professional intervention but do not require hospital admission. Citizens can receive a stay to prevent hospital admission or to recover and become ready to return home.

As of January 1, 2027, 202 employees from nine municipalities will transfer to the center. The allocation unit will become part of the center’s staff function, and you will report directly to the center chief.

We are in the process of building a new organization, where we will jointly develop workflows, collaboration relations and solutions that support a coherent and citizen-near health service.
