Team Leader for New Health and Care Spaces with a Focus on Strong Relationships and Cross-Sector Development

Do you want to lead the establishment and development of our new Health and Care Units (SOP) on Stenhusbakken in Holbæk town?

Do you have the professional drive to lead complex patient pathways, and are you motivated by building bridges between municipality, hospital and general practice? Then you may be the team leader we are looking for.

From 1 January 2027 we will convert 19 of our current temporary placements into Health and Care Units, which we operate in close collaboration with the Region and Holbæk Hospital. Our current team leader has chosen to seek new challenges closer to home, and we are therefore looking for a visionary and relation-strong nursing-qualified team leader who will lead this exciting transition.

About the leadership team and our culture at Stenhusbakken

You will become part of a dynamic leadership team comprising two team leaders and a center leader. You report to the center leader of Stenhusbakken. We place high value on close collaboration and we spar closely with each other in everyday life.

About the position

You will become the leader of a professionally strong staff group where we currently have 15 healthcare assistants (SSA), 7 social and health assistants (SSH), 6 nurses and 2 therapists. This group must be further supplemented in connection with the transition to SOP

As team leader for the SOP units, you will have a leadership role that extends far beyond daily operations. The SOP area is rapidly developing, and you will become a central figure in shaping and adapting future workflows and cross-sector patient pathways.

Your main tasks will be to

• Ensure high professional quality: Create a robust and safe professional environment around patients with complex, acute and often changing needs.
• Drive cross-sector collaboration: Facilitate and strengthen coordination and relationship-building across Holbæk Municipality, Holbæk Hospital and general practice.
• Lead professional development: Thrive in a dynamic field where workflows and collaboration forms continuously need to be developed, evaluated and adapted.
• Staff management: Exercise close leadership for an engaged employee team, where you support well-being, professional sparring and strong interdisciplinary cooperation.
• Take leadership and create understanding: Make the necessary and sometimes difficult managerial decisions, and communicate them in a way that creates meaning, ownership and understanding in the staff group.

We are looking for a profile who

• Has strong professional and relational competencies: You use your strong health professional background as a nurse to spar with staff about complex pathways.
• You stand solidly in your leadership role and have previous experience with personnel management. It would be an advantage if you have a leadership education, but not a requirement.
• Are a strong communicator and bridge-builder: You are able to balance the professional and the relational, and you manage to get different professional groups and organizations to work towards common goals. You create followership through trust, openness and an inclusive leadership style.
• Are development-oriented: You are motivated by change processes and can navigate safely in a field where the framework and tasks are not always given in advance.

Salary and employment terms

The position is a full-time role with start date 1 January 2027.

Salary and employment terms are set in accordance with the applicable collective agreement and the principles of Ny Løn.
