Become a Route Planner in the Integrated Care in Gribskov Municipality

Do you want to combine your health professional experience with sharp planning and modern digital tools? As a route planner in the Integrated Care you will have a key role in making everyday life run smoothly for both citizens and colleagues – with 37 hours per week, close collaboration and a workday where your efforts can be felt directly with the citizens.

You will become a central part of daily operations, where based on citizens' authorized services, time windows and individual needs you help ensure that resources, quality and economy come together in one higher unit. You will work closely with management, the duty scheduler, other planners and field staff – and you will have great influence on planning your own work day.

Your key competencies in the job as a route planner

• You have a strong health-professional profile and are educated as a Social and Health Assistant or nurse – preferably with experience as field staff.
• You are familiar with Nexus, competency levels and working with authorized services and citizen pathways.
• You have knowledge of the Health Act, the Social Services Act and the Elderly Act and can translate legislation into practice in the planning.
• You are analytical, structured and detail-oriented and can keep an overview of many tasks at once.
• You are flexible, adaptable, robust and able to decide – and good at prioritising when things change.

Your central tasks in route planning

• Develop and optimize daily and future driving lists and routes focusing on efficient use of resources and economy.
• Ensure that planning takes into account citizens' authorized services, time windows and individual needs.
• Distribute tasks considering employees' competencies, working hours, geographical location and the financial consequences of the planning.
• Adjust plans in case of illness, holidays, urgent tasks or other operational changes with focus on efficient and economically sensible operation.
• Collaborate with relevant partners and external suppliers.
• Actively work to reduce driving time, unnecessary transport and other costs and support efficient use of staff resources.
• Assess the financial implications of different planning solutions and choose solutions that create the best balance between citizens' needs, employees' working hours, quality and economy.
• Monitor planning quality, resource consumption and financial efficiency and suggest improvements of workflows.
• Monitor changes in citizen pathways and continuously adjust plans considering both operations and economy.

The position is 37 hours with start on 1 January 2027.
