A clinically strong nurse as clinical coordinator in the day shift for Jægersborghave, 2nd floor south

Then we are looking for you as our new clinical coordinator during daytime shift as of 1.2.27.

You will work every 6th weekend. The working hours are from 7:30 to 15:00. The position is 37 hours per week, and together we will find a rota that supports a good balance between work life and private life.

About the position

As clinical coordinator you will be part of a team consisting of a department head, another clinical coordinator in the day shift, two on-call managers in the evening shift, and dedicated colleagues with different professional backgrounds. You collaborate closely with the department head and the other staff through both individual and group sparring.

You will be responsible for the nursing coordination of 12 residents and contribute to the daily care of residents with both basic and complex care needs. The role includes, among other things:

• Planning and coordination of nursing care interventions.
• Holding and follow-up on nursing home doctor visits.
• Interdisciplinary collaboration with doctors and other partners.
• Documentation and follow-up in Nexus.
• Collaboration with relatives.
• Responsibility for medication management and the medication room.
• Quality assurance and follow-up on professional interventions.

A versatile and practice-oriented position

The position provides the opportunity to work broadly within nursing. You will alternate between basic, complex and palliative pathways and play a central role in ensuring that residents experience safety, dignity and quality of life throughout their stay.

We have strong interdisciplinary resources in the form of a dementia consultant and development nurse, who support the professional development in the department.
