About the job

Om jobbet

Nurse with completed APN education

We are seeking a dedicated nurse with APN training to support and develop work with citizens with multimorbidity and complex issues in the Municipality of Aarhus – at both clinical, citizen-facing levels and through identifying patterns and trends that can contribute to development and quality assurance of the citizen-facing practice.

In the Department of Health and Care (MSO) our foremost task is to help the citizens who need nursing and health care assistance so they can maintain the life they wish for. The keywords are rehabilitation, multidisciplinary collaboration, quality and continuity.

The task is changing and requires that we stay ahead and incorporate the latest knowledge

  • Case trajectories are becoming more complex and require novel solutions
  • The solutions require a holistic and multidisciplinary focus
  • Collaboration across sectors is becoming increasingly important
  • Citizens’ own and their relatives’ efforts are prioritized higher and higher

In MSO we have a team consisting of six APN nurses, who solve a wide range of tasks related to especially complex citizen trajectories. You will become part of this team, and you will have the opportunity to draw on and build collaborations internally in MSO, cross-municipal, cross-sectoral and nationwide.

Tasks vary widely. Some tasks you will need to seek out and drive yourself. Others you will develop together with colleagues mono- or interprofessionally.

A typical citizen pathway starts with a request from staff or management, and together with them you tailor an individually adapted intervention for the citizen. Your role as APN nurse can be either as a sparring partner or with hands-on involvement in daily care.

The length of the pathways varies greatly, and there can be both week-long pathways with APN holistic assessment, including objective examination, follow-up, or shorter pathways of a more supervisory nature.

The citizens APN nurses most often assist are citizens with multimorbidity with frequent (re)hospitalizations, polypharmacy and reduced health competencies. In addition, citizens with dual diagnoses and substance abuse issues occupy a substantial part of our work. There is also often organizational complexity in the form of many health professional contacts across sector boundaries. The problems collectively create a particularly complex situation where you as an APN nurse help to create overview and flow in the process, while you are present in everyday life close to the citizen, staff and any relatives.

In addition to practice-oriented citizen pathways, you as an APN nurse will also have the opportunity to participate in project-oriented work. Since the APN function is still rapidly developing in MSO, you will have here an opportunity to be part of the journey and to influence the solution of tasks related to the particularly complex citizen pathways.

We would like to point out that the position lies within the 24/7 area. Currently the service is expected to be primarily day shifts.

Your qualifications

You have completed the master's program in ”Advanced Clinical Nursing” (Advanced Practice Nursing) and have a minimum of 2 years of clinical experience as a nurse in primary or secondary sector.

You are a person who

  • Has a sharp clinical eye and a strong theoretical foundation
  • Is interested in citizens with multimorbidity and diverse problems
  • Is interested in cross-sector and cross-disciplinary work
  • Coordinates and organizes nursing interventions based on the citizen's and relatives' wishes
  • Thrives in both independent work and in collaboration with others regardless of professional background and function
  • Enjoys being “a guest in the house” and traveling between several teams and naturally entering into new professional relationships
  • Thrives in the presence of others and sees their potential
  • Is interested in translating theoretical knowledge into new and improved practice
  • Can take a leading role with partners and delegate shared responsibility
  • Possesses strong communication skills and can think creatively in complex situations
  • Can organize your own workday and seek sparring when needed
  • You must have a driver's license

About the department

It is important to us that the approach and knowledge you bring with you are shared across the organization to all 6,400 employees and for the benefit of all citizens. Therefore APN nurses work across the three citizen-facing administrations.

You and your APN colleagues currently have office space together in Risskov, where you meet several times a week for professional discussions about citizen pathways, sparring and collegial solidarity. The concrete citizen pathways are distributed across the municipality, so your base location may vary from day to day. You will report to the manager, Mette Riisberg Evald.
