Care Pathway Coordinator for Neurorehabilitation in Grindsted and Lunden Rehabilitation Center, Varde, for Employment Starting 1 January 2027

Are you passionate about neurorehabilitation, interdisciplinary collaboration, and cohesive patient pathways? We are seeking a care pathway coordinator for Rehabilitation for Brain and Nerve Diseases Grindsted and Lunden Rehabilitation Center, Varde (hereafter referred to as Lunden). As part of the Health Reform, the authority for specialized level rehabilitation will be transferred to the regions on 1.1.2027. The most specialized part of advanced-level rehabilitation will also be transferred.

In this context we are looking for a Care Pathway Coordinator; an occupational therapist or physiotherapist in a newly created position who, in collaboration with the Visitation and the multidisciplinary team around the patient, contributes to ensuring a cohesive and well-coordinated patient pathway across sectors.

Who are we We receive patients with acquired brain injury who require interdisciplinary and specialized neurorehabilitation. The effort is based on high professional quality, systematic knowledge sharing, and close multidisciplinary collaboration to create cohesive pathways that strengthen patients’ functional ability, quality of life and the possibility of an independent everyday life.

About the role As a care pathway coordinator you will play a central role in ensuring coherent, efficient, and consistent pathways for patients in neurorehabilitation across Grindsted and Lunden.

You will be a key person in complex pathways where several professional groups, departments and sectors are involved, and where collaboration in transitions between regional and municipal levels is crucial for a good patient journey.

The position is organizationally placed under the neurological department management of Brain and Nerve Diseases, and reports scientifically to the Head Therapist in Rehabilitation for Brain and Nerve Diseases, Grindsted.

Your tasks As a care pathway coordinator you will in collaboration with the departments at Grindsted and Lunden contribute to:

• Coordination of complex patient pathways across Grindsted, Lunden, other regional and municipal offerings with focus on overview, structure and cohesive transitions.
• Visitation and professional sparring about rehabilitation level, interventions, goals, prognoses and completion of pathways in collaboration with relevant professionals and Visitation.
• Capacity management and patient flow with an overview of occupancy, prioritization and appropriate use of rehabilitation offerings across sites and external providers.
• Cross-cutting collaboration and communication between professional teams, sectors, partners as well as patients and relatives to ensure timely and well-coordinated transitions.
• Quality assurance and development of rehabilitation offerings, rehabilitation plans, outgoing rehabilitation pathways and common workflows.
• Competence development, documentation and administrative tasks in collaboration with colleagues, Visitation and management, including contributing to the development of instructions and collaboration structures.

Your profile We are looking for a profile who

• Is a licensed occupational therapist or physiotherapist with Danish authorization
• Has neurological knowledge and experience as well as experience with neurorehabilitation
• Has experience with municipal collaboration and patient pathways across sectors
• Is familiar with GOP criteria for rehabilitation at a specialized level when transitioning to municipal administration
• Has good collaboration and communicative skills, both internally and with regional and municipal partners
• Has experience with teaching, guidance and professional sparring
• Thrives on creating overview, structure and direction in complex pathways and on working across disciplines and sectors

Employment terms The position is a full-time role at 37 hours.

Employment is conducted according to the applicable collective agreement. Qualification and function allowances are negotiated with the department management.

The workplace is Rehabilitation for Brain and Nerve Diseases Grindsted, with expected physical and/or virtual presence at Lunden in Varde, with external providers and at relevant status meetings and conferences.

It is necessary to have a driving license.

You will be told the base salary, centrally fixed allowances and pension in Ergo-, physiotherapists, and midwives - 33.21.1 - salary as of 1.10.2026 .
