We are seeking a social- and health care assistant for a permanent day shift position (30-37 hours per week) with work every 3rd weekend for our citizen team with base in Hornslet. Be part of creating more self-determination – for both citizens and employees

Our interdisciplinary and self-managing citizen teams work to create greater continuity and quality in care – as well as more self-determination for both citizens and employees.

We consist of two teams covering respectively the Hornslet area and Mørke and the surrounding villages. In total we provide both help and support under the Social Services Act, the Elderly Act and the Health Act, and we work with a holistic approach based on the citizen's needs and wishes.

Our teams are interdisciplinary and consist of social- and health care assistants, social- and health care aides, nbsp; nurses and substitutes. In addition, we have a fixed visitor and a therapist attached.

Together we plan, coordinate and perform care and nursing tasks with holistic care as the common focal point and with a focus on professionalism, rehabilitation and self-determination. The close daily coordination and the shared responsibility in the team ensure high quality and greater coherence for the citizens – and at the same time give each employee great opportunity for influence and professional development.

As a social- and health care assistant in the team you will play an important role in coordinating and developing the care, and you will help ensure that the professionalism is at the top – both in daily operations and in long-term development.
