About the job

Would you like to help shape the future of the local health care system?

As a result of the Health Reform, the health and care placements will transfer to regional responsibility and operation as of January 1, 2027. The health and care placements are targeted at citizens who require an intensive health professional intervention such as rehabilitation, observation, treatment and care, but who do not require hospitalization.

The task is anchored at North Zealand Hospital in a new organizational unit under the Department for Elderly and Multimorbidity and includes seven geographical locations in North Zealand. In Halsnæs there will be 18 health and care placements, located at Søndergade 81A in Hundested.

Here we will bring together strong health-professional and rehabilitative efforts around citizens with complex needs, and especially for the health and care placements in Halsnæs, we will build a completely new team of occupational and physiotherapists. You will therefore have a unique opportunity to help shape the development of a new regional service and the establishment of a strong team-based and interdisciplinary environment with a focus on the good and coherent patient journey.

As the region first takes over responsibility for the upcoming health and care placements on January 1, 2027, you will start your employment in the Department of Elderly and Multimorbidity at Frederikssund Hospital on December 1. Here you will receive a thorough introduction to the patient pathways, and you will learn to use the Health Platform, which is the electronic medical record system that will be on the health and care placements from January. From January your permanent workplace will be in Halsnæs.

About the position

As an occupational therapist, you will play a central role in

  • Plan and implement targeted rehabilitation and reconditioning pathways in close interdisciplinary collaboration.

  • A strong interdisciplinary team collaboration with nurses, social- and health care assistants, social- and health helpers, physiotherapists, doctors, case managers and other relevant partners.
  • A workplace with a focus on rehabilitation, professional quality and citizen involvement.
  • Good opportunities for competency development and professional sparring.
  • Visible and approachable leadership.

We are seeking an occupational therapist who

  • Has experience with rehabilitation of older adults and citizens with complex health-related problems.
  • Can translate rehabilitation into practice and contribute to citizens experiencing a coherent, meaningful and targeted pathway.

  • Thrives in a multidisciplinary environment and actively contributes to team collaboration with other professional groups.
  • Is independent, proactive and adaptable.
  • Has strong communication skills and is able to create security and motivation for citizens and relatives.
