Occupational therapist or physiotherapist for the Holistic Care Group in Assens

Are you passionate about creating coherent citizen pathways through cross-disciplinary collaboration and based on the citizen's motivation, goals, wishes and needs? And do you want to work with rehabilitation within the Elderly Act and the Service Act? Then you might be our new colleague!

The Holistic Care Group in Assens is seeking an occupational therapist or physiotherapist who is passionate about working with rehabilitation together with a multidisciplinary team consisting of social and health care assistants, visitator, case-handling therapist, training assistants, pharmaconomist, nurses and a few unskilled staff, who together create streamlined workflows with as little bureaucracy as possible.

There is, in principle, a full-time position of 37 hours per week, but the number of hours can be discussed at a possible interview.

The position is intended to be filled as of January 1, 2027.

About the job In the holistic care group we work both day and evening shifts. Many of your future colleagues are at work from 7:00 in the morning, and the last leave at 11:00 p.m. You will as a therapist be employed as self-directed, and in order to take clinical leadership it is therefore important that you meet with your multidisciplinary colleagues. This means for you that in your planning you must ensure that you meet as much as possible with your colleagues within their meeting times. As a field-based group there will be a need for you to sometimes meet at 7:00 a.m. on certain days.

About you Your professional competencies are based on

• Solid insight into the rehabilitative mindset and the concept of rehabilitation
• Knowledge of and experience with function assessment and activity analysis
• Working with citizens' motivation and the zone of proximal development
• Familiarity with one's own and others' monofaglige competencies
• Awareness of different ways to obtain information about the citizen's life situation, functional level and development in meeting with the citizen
• Seek knowledge about opportunities in civil society based on the citizen's motivation
• Competencies in co-creating and problem-solving communication as well as motivational interviewing
• Commitment and motivation to develop yourself and the area
• Overview; ability to see wholes and cross-disciplinary connections and develop a rehabilitation plan that supports the citizen's development
• Structure and facilitate citizen pathways
• Communicate clearly and respectfully, both in writing and orally, with awareness of the target audience for the communication
• Use relevant IT systems as a natural working tool
• Apply local and national requirements and guidelines for necessary and correct documentation
• Professional sparring with colleagues and thereby bring their insights into the citizen's condition and daily life into play
• Be an important catalyst for progress and reflection in the citizen's rehabilitation process
• Take part in the cross-disciplinary clinical leadership in daily work

About us The Holistic Care Group in Assens is the largest of four field-based holistic care groups in Assens Municipality. We are about 100 employees consisting of nurses, social and health care personnel, therapists, a few unskilled staff, training assistants, a visitator and an assistant manager. We work with the Health Act, the Elderly Act and the Social Services Act (§83, §83a and §112).

The monofaglig development is the focus across the holistic care groups with a view to improving ourselves so that we become even stronger in cross-disciplinary collaboration in each group. We look forward to telling you much more about us at a potential interview.

Salary and terms of employment Salary and employment are according to the applicable collective agreement and based on the principles of local pay formation.
