About the job

Do you have an eye for both citizens' needs, employees' competencies and the good use of our resources? And do you thrive in creating overview, coherence and calm in a daily life where many considerations must add up?

Then we have an exciting position as planner in the Integrated Care in Præstø.

In the Integrated Care we work to create cohesive citizen pathways and strong multidisciplinary teams around the citizens. As a planner you will have a central professional and operational role, helping to make daily life hang together for both citizens, employees and management.

About the job

Your main task is to ensure a coherent planning of both home care and nursing for the team you are primarily attached to – in both day and evening shifts.

Planning for us is more than just placing visits on routing lists. You should create a balance between professional quality and patient safety, the citizens' needs and continuity, realistic routes, employees' competencies and sustainable operations. You will work closely with the team, area manager and the area's other planner and contribute to making the planning stable, forward-looking and usable in practice. We are in the process of implementing a digital route planning tool that can assist you with the planning task.

Your main tasks

  • Prepare and continuously maintain standard plans for both home care and nursing in day and evening shifts.
  • Plan citizen visits with regard to professional requirements, competencies, continuity, the citizen's needs, geography and efficient resource use.
  • Work systematically with 5–7 days ahead and handle known changes, absence and changes in citizen paths as early as possible for the effort.
  • Handle necessary day-to-day changes in the routing lists for SSH/SSA in close cooperation with the team and help ensure coverage in case of acute absence.
  • Participate in relevant shift-start and triage meetings and translate professional assessments into concrete adjustments in planning.
  • Coordinate the planning start-up of new citizens and ensure that changes in the citizens' needs are reflected in the planning.
  • Maintain ongoing dialogue with the area manager about capacity and staffing needs and cooperate closely with the on-call planner and the other planner in the area.
  • Contribute to common and uniform working procedures for planning in the Integrated Care.

Who are you?

We are looking for you who can create structure and overview in a complex operation and at the same time have an eye for the professional quality in planning.

  • You have a healthcare background with good knowledge of the Elderly and Health Act.
  • You understand the competencies of different professional groups and can ensure that tasks are placed with employees who have the right competencies.
  • You can prioritize and make well-founded decisions, even when everyday life changes quickly.
  • You communicate clearly and respectfully and thrive in close collaboration with staff and management.
  • You work systematically and have good IT skills. Experience with planning or disposition is an advantage, but not a requirement.

A central role close to everyday life

As a planner you will have great influence on how everyday life works for both citizens and staff. You will become part of a development where the Integrated Care strengthens a common, professionally grounded and more forward-looking planning practice. You report to the area manager and will have close interaction with both area managers in Præstø.

We offer

  • A central function with great significance for daily quality and operation.
  • Close collaboration with dedicated colleagues, planner colleague and management.
  • Opportunity to help develop the way we work with planning.
  • A daily life with both professional, coordinating and operational challenges.
  • Introduction and support to help you settle well into the role.

Practical information

  • The position is 37 hours per week
  • Working hours will mainly be on weekdays within the period 6:30 to 15:30
  • Start date: January 1, 2027
  • Application deadline: November 18, 2026
