Social and Health Care Assistant for an outreach team at Kildemarkscentret, District South-East

We are seeking a good colleague for a strong multidisciplinary community, where you with your professional expertise will help contribute to holistic care, interdisciplinary collaboration and citizen involvement. We work in fixed multidisciplinary teams, where continuity, presence and high quality in citizen pathways are at the center.

The team operates from Kildemarkscenteret, and visits citizens in their own homes.

The team is multidisciplinary and we work with the improvement model, hold daily triaging meetings, where you as a social- and health care assistant will also take a central role and are expected to apply your professional knowledge.

We work from the perspective of the Elderly Act with focus on holistic care, employee continuity and the citizen's self-determination. Rehabilitation and prevention, early detection of dementia, acute nursing and palliation are our professional foundation. We work citizen-centered, and are happy to go the extra mile to ensure the best possible effort for the citizens.

About the position

As a colleague in the team, you will be part of helping to ensure continuity, presence and high quality in the citizen's everyday life.

We work every 2nd weekend.

A criminal record check will be obtained upon employment and it is nice if you have a driver's license.

Your tasks

• ensure quality and continuity in the citizens' care and treatment
• perform tasks according to competence and delegation
• coordinate and actively participate in our multidisciplinary meetings
• you will receive good onboarding in Nexus.
• create overview of citizen pathways and contribute to planning of the day's tasks
• engage in close collaboration with colleagues, nurses and therapists
• contribute to professional sparring and development in daily life

Your profile

• you are trained as a social- and health care assistant
• you are professionally well-founded and can reflect on your own practice
• you are eager to take responsibility and work independently
• you have a good clinical eye and work from a holistic perspective
• you thrive in a coordinating role and in close collaboration with nursing
• you contribute to a good atmosphere at the workplace.

Working hours per week 37 hours per week in mixed shifts, of course with the understanding that it can work for everyone. The number of hours can be negotiated.
