The position is 37 hours per week with primarily day shifts, evening shift 2-4 times per month and day shift every 2 weeks on even weekends.

Blomstergården is a modern care and rehabilitation center with 47 temporary placements distributed across four fixed teams. Here you will meet approximately 80 dedicated colleagues who every day work to create dignified, coherent and meaningful citizen journeys.

Your tasks

• Holistic care and nursing-related interventions
• Observation, assessment and follow-up on citizens’ condition
• Medication management and documentation
• Coordination and interdisciplinary collaboration around citizen journeys
• Rehabilitative interventions focusing on the citizen’s own resources
• Guidance and support for citizens and relatives
• Person-centered approach and pedagogical action plans
