Social and Health Care Assistant for Evening Shift in Holistic Care

In the Integrated Care Team (Helhedsplejen) in Gribskov Municipality we are looking for someone who has a passion for people, development and an exciting work life in the evening shift – with the possibility of flexible shifts and up to 37 hours per week.

We are recruiting in Gilleleje, Græsted and Helsinge.

You will become an important part of our Integrated Care Team, where we work in a rehabilitative way in small teams, close to the citizens and with a focus on both quality and presence in the evening hours.

Flexibility is offered in hours and rosters, and there is the possibility of 12-hour shifts from 11 a.m. to 11 p.m. when it suits you, the team and the operations.

Your key competences

We would like you, as a social and health care assistant in the evening shift, to be able to tick off among other things

• a solid professional level and a desire for professional development
• good and systematic documentation that supports quality in citizen pathways
• strong relational competences and the ability to be a clear and loyal team player
• experience with or interest in interdisciplinary collaboration
• confidence with IT and digitalisation, including use of the journal system NEXUS.

Your central tasks in the evening shift

As a social and health care assistant in the evening shift in the Integrated Care Team you will among other things

• actively participate in team collaboration.
• understand and take responsibility for your own role in the Integrated Care Team and in the team
• be a clear and trustworthy relationship builder towards citizens, relatives and colleagues
• contribute to rehabilitative interventions with focus on the citizens' resources
• contribute to interdisciplinary discussions and professional reflection in everyday life
• document and work digitally, so information is shared and used effectively

The position is up to 37 hours with start by agreement

Employment is carried out in accordance with the applicable FOA agreement between KL and Gribskov Municipality's wage policy. The agreement can be read at www.kl.dk
