We are seeking a social and health care assistant for 32-37 hours per week for Home care in Espergærde.

We help each other when everyday life becomes busy, brainstorm about the professional challenges and take time to follow up with each other and our citizens. We believe we solve the task best when we use each other's competencies and stand together on what may be difficult.

The good collaboration is also a part of our structure. Every morning we meet for team coordination , where we follow up and allocate the day’s tasks according to our competencies. In addition, we have three triage meetings and two interdisciplinary meetings per week across day and evening shifts .

This means that we continuously follow up, share our observations and ensure that the tasks are carried out with the right competencies.

A good start is important: When you start with us, you follow an experienced colleague with the possibility of on-the-job training and ongoing sparring .

You will also be enrolled in relevant internal courses that prepare you for your tasks.

We want you to feel safe and well equipped before you stand on your own.

Your professionalism makes a difference.

As a SSA you will have responsibilities, among other things for

• Health-related tasks and care.
• Observation and assessment of changes in the citizens' condition.
• Medication management and follow-up.
• Documentation and professional coordination.
• Interdisciplinary collaboration and rehabilitation.

We are looking for you who are professionally engaged, responsible and able to work independently .

You have a sense of the citizens' needs and actively contribute to good collaboration.
