Social and Health Care Assistant at Solgården

At Solgården we are looking for an engaged social- and health care assistant who will make a difference for people with mental disorders, substance use dementia and late-brain injury. You will become an important part of the citizens' daily life, where relationships, trust and respect are essential.

Your role as a social- and health care assistant will be to work with the health- care aspects, the rehabilitative and relational interventions with the focus that the individual citizen can master as much as possible in their own life and maintain as much self- and co-determination as possible.

Your tasks include, among others

• medication management and follow-up on treatment
• health- care observations, early detection and prevention
• collaboration with doctors and other health- care partners
• holistic care and support for daily activities
• psychiatric and somatic issues
• dDocumentation in KMD Nexus
• contact person role and coordination around the citizen
• professional sparring and knowledge sharing with colleagues, students and trainees.

Working hours are variable and include both day and evening shifts in a 4-week rotation.
