Social- and Health Care Assistant sought for Thisted Clarification Center – with opportunity to help create the new Health and Care Places

We are looking for a Social- and Health Care Assistant who wants to become part of our team at Thisted Clarification Center.

You will be employed at Thisted Clarification Center with start on December 1, 2026 . From January 1, 2027 the clarification center will be transferred to Region Nordjylland, and we will become part of the new Health and Care Places .

At Thisted Clarification Center we work with citizens who need care, health interventions, rehabilitation and clarification.

With the transition to Region Nordjylland and the establishment of the new Health and Care Places, we are facing an exciting development.

We therefore seek you who want to be an active part of this process. You should be able to see the possibilities of helping to create something new, while focusing on the individual citizen's needs, safety and quality of life.

You will become an important part of an interdisciplinary environment where we collaborate to create good and cohesive citizen pathways.

Your daily life with us

As a Social- and Health Care Assistant you will, among other things

• Provide holistic care and support based on the individual citizen.
• Perform health-related tasks and observations.
• Observe, assess and respond to changes in the citizen's condition.
• Work rehabilitatively and support the citizen's own resources.
• Document and follow up on the citizen's development and health-related interventions.
• Create safety and continuity for the citizen.
• Engage in close collaboration with citizens, relatives and interdisciplinary partners.
• Contribute actively to the development of the new Health and Care Places.
• Share your expertise and help create a strong professional culture.

Who are you?

• Is educated as a Social- and Health Care Assistant.

It is advantageous if you have experience from for example the elderly care sector, rehabilitation, clarification centers or working with complex citizen pathways. However, the most important is that you have the desire and motivation to be involved.

Practical information

• Place of employment at start: Thisted Clarification Center, Thisted Municipality.
• The position: Typically 37 hours per week.
• Working hours: Primarily day and evening shifts. There may be coverage in night shifts when needed.
• Weekend: Every other weekend.
• Start: December 1, 2026.
• Salary and terms: According to the applicable collective agreement.
