We are in the process of consolidating the dementia efforts at Plejecenter Glesborg and developing a strong professional environment.

The care center consists of both a dementia department and a general department. In addition, there is an associated dementia day center with its own staff, which supports an active and meaningful everyday life for the residents.

As part of the overall dementia effort, there are two dementia coordinators attached, who work with citizens in their own homes in Norddjurs Municipality.

The dementia coordinators also provide professional sparring and guidance to staff at the municipality's nursing homes.

To support the development of our shared culture and collaboration, we are undertaking a targeted course program focused on team development, collaboration and a common professional direction. At the same time, we prioritize ongoing professional development so that we can together create the best possible care for our citizens.

Your responsibilities

• Care, support and pedagogical assistance to people with varying needs
• Relationship work with focus on presence, security and recognizability
• Work based on the residents' daily rhythm plans and life histories
• Support residents' self-determination and participation in everyday life
• Documentation in Nexus
• Active participation in the development of professional practice
• Participation in triage

• Is educated as a social and health care assistant
• Works responsibly, reflectively and professionally with care, relationships and documentation
• Thrives in interdisciplinary collaboration and contributes positively to the community
• Is eager to help develop the dementia effort of the future at Plejecenter Glesborg
• Sees the value of learning, development and interdisciplinary collaboration

Working hours

• Primarily day shifts
• Work every other weekend
• Full-time
