Social worker for at-risk youth – Housing First and substance abuse assessment

Do you want to work with "Housing First" and addiction referrals at the Youth Center Brønderslev?

UngeCenter Brønderslev is looking for you for a permanent full-time position with start as soon as possible and preferably no later than December 1, 2026.

We are looking for a new colleague for the specialized team at the Youth Center, targeting the population of substance abuse and homelessness. The position is a combined role, where your working time will be distributed about 70% on our "Housing First" initiative and 30% on referring citizens to addiction treatment.

Housing First: We want to strengthen the efforts to prevent homelessness and to seek care placements. It is especially important for the young person to ensure sufficient resources to quickly work on establishing and providing the necessary support in their own housing if the young person has stayed at a care home. Our new advisor will implement support according to established methods such as CTI, ACT and ICM, but also be willing to develop new practice.

Substance abuse referral: You will be responsible for referring young people aged 18-29 to addiction treatment. We expect that you have knowledge of the typical substance use issues and mental health challenges of the youth group.

Who are we? UngeCenter Brønderslev is a multidisciplinary unit aimed at all 15-29 year olds in the municipality. Our core task is to help and retain young people in work and education by ensuring a coherent holistic approach. You will be assisted by colleagues and professional consultants with specialized knowledge in the areas, so even if you are primarily responsible for the "Housing First" initiative and the addiction referrals in the team, you will not be alone.

Your authorities duties on both areas will be

• Referral and follow-up in cases of addiction treatment.
• Assessment of combined or integrated pathways
• Exploration of inter-municipal reimbursement
• Advice and guidance to young people and their families.
• Participation in network meetings.
• Case handling under the Social Services Act sections 85a, 85, 101, 102 and 110.
• Case handling under the Health Act sections 141 and 142
• Participation in professional sparring, well-being meetings, expert group and supervision.

We expect you to have the following professional competencies

• A social work degree.
• High professional competence, where you are able to engage in dialogue with young people with complex challenges.
• Knowledge of employment and the Social Services Act and preferably experience in the field of substance abuse.
• Good collaboration skills.
• Familiarity with Nexus and/or Momentum is an advantage.
• Driving license

Salary and terms of employment: In accordance with the applicable collective agreement between KL and the negotiating party organization. A police certificate and child protection and criminal record check will be obtained before employment.
