SSA for route planning in home care

• We are a district that is well on its way with the implementation of the new elderly act and have established 3 well-functioning multidisciplinary teams, which are the central point for holistic care and coherent citizen pathways

• You will become a central part of our route planning team, where you will have the opportunity to use your SSA expertise and your knowledge of citizen needs in a coordinating role, where you contribute to ensuring that citizens receive the help they need

The job and your main tasks

• Route planning and maintenance of standard plans
• Coordination of emergency calls and urgent changes to the plans
• Coordination of start-ups and changes in citizen pathways, where you contribute to creating a balance between the citizen's needs and the professional competencies and ensure continuity and coherence in the routes
• Daily coordination of staffing needs with the shift planner and supporting manager to ensure the best possible use of resources
• Collaboration with the Authority Department regarding new visits and follow-up on ongoing efforts and pathways with the 3 teams
• Telephone contact with citizens and relatives
• Handling of citizen-related tasks in Nexus
• Participation in relevant meetings in the 3 teams
• Carrying out onboarding meetings with new citizens
• Contributing to the implementation of new workflows and development of the planner function
• Most of your hours will be full days in the planning function, but there will also be full days when you must work as an assistant in operations

You are

• Social and Health Care Assistant with high professional standing and knowledge of home care
• Structured and able to priorities and maintain an overview, even when everyday life changes and solutions must be found along the way
• Capable of juggling several balls in the air at the same time
• Loyal to the municipality's quality standards and service level
• Responsive, good at collaborating and able to maintain a good tone in dialogue
• Skilled in documentation, accustomed to IT and eager to learn new systems
• Responsible in relation to operations and contribute to prudent use of financial resources
• Curious and willing to develop both yourself and our work processes and collaboration.

Practical information

The position is 37 hours per week and is desired to start on 1 December 2026 (we are flexible if you want a lower number of hours).

Working hours are alternately 6:30-12:30 and 6:30-15:30 on weekdays.

Weekend shifts every 4 weeks and holiday rotation with the other route planners – weekends and holidays, the route planners work as SSA on an equal footing with colleagues.

The route planner team covers each other during holidays and time off.

Salary and employment terms in accordance with the applicable collective agreement.
