We are experiencing growth, and Svane Pleje is therefore currently seeking 2 social and health care assistants for the evening shift for our department in Syddjurs.

We are looking for a colleague who can work independently and provide the best care and support to our citizens. We offer a permanent position in our team of evening shifts with an average of 28 hours per week and with the possibility of more hours.

As a social and health care assistant in the evening shift you will provide care and practical assistance to our citizens in their own homes. At Svane Pleje we work rehabilitatively in close collaboration with citizens and relatives and do everything to create the best conditions for our citizens – with particular focus on their life situation. We therefore have a strong focus on developing internal workflows and making time and space for collegial collaboration.

Participation in teams and planned time for sparring is therefore one of the activities in everyday life that we want everyone to participate in and take responsibility for – and you will thereby be able to influence the team with your initiatives.

The tasks are primarily

• Basic care and nursing tasks
• Practical help and personal care that supports the citizen in maintaining their dignity and creates joy in life
• Observation of citizens with a view to early detection of illness with the aim of reporting it to our partners in the respective municipality.
• Collaboration with citizens, colleagues and municipal partners
• Documentation in the care system CURA

Your primary workplace is in Syddjurs Municipality with attendance at our office in Hornslet, where there is the possibility to borrow one of our company cars that are available.

You will become part of a team of assistants, carers and relief workers overlapping with day and night shifts. We work value-based and are motivated by inclusion and influence.
