Nurses wanted for Health and Care Placements in Fredericia, Nær Sundhed, Lillebælt Hospital

Would you like to be part of a very exciting task related to the implementation and establishment of the new Health and Care Places in Fredericia, so we are now seeking 3 nurses.

We are seeking 3 licensed nurses who are passionate about complex citizen pathways, rehabilitation and the interdisciplinary collaboration in the close health care system.

The positions are initially 37 hours/week; if you prefer a lower number of hours, there is also an option for that. You will join a duty roster with weekend shifts, and the positions are in mixed day and evening shifts.

Health and Care Places Fredericia The staff group consists of social- and health-care assistants and helpers, nurses, occupational therapists, physiotherapists, house assistants, and medical coverage from hospital and general practice.

Our core task is to receive citizens with complex needs for specialized nursing, rehabilitation and palliation of a nature that cannot be solved at home, and a main task will be to bridge the gaps between sectors.

Citizens must be rehabilitated through close interdisciplinary collaboration, so that citizens’ resources and health skills are strengthened, and thus regain their independence and a higher quality of life.

There will be close cross-sector collaboration with the acute nursing service in Near Health, general practitioners, responsible treatment wards, training and municipal help and nursing.

The goal is that multimorbid citizens safely return home, and that readmissions are prevented.

About the position As a nurse at Health and Care Places in Near Health, the core task is to ensure that citizens with complex health needs receive a coherent, professionally qualified and safe follow-up in the aftermath of or as an alternative to hospitalization. The core task is to strengthen functional ability and well-being for the individual citizen.

You will follow a structured and well-planned introduction program and be assigned a mentor who takes responsibility for your introduction. You will continuously have good opportunities to develop your competencies during your employment.

About the department The Health and Care Places in Fredericia are located at Viaduktvej 9B, 7000 Fredericia, and are part of “Near Health” - a new department at Lillebælt Hospital. Salary and employment conditions Salary and employment terms for the position follow the Health Collective Agreement – Agreement 32.17.1 - salary as of 1.4.2026. In addition to the starting salary, the pay may include negotiated allowances depending on the position, your experience and qualifications.
