About the job

Do you want to help create safe, coherent and timely discharges?

A good discharge starts long before the patient leaves the hospital. It requires clinical understanding, overview and close coordination between the patient, the hospital and the municipal partners.

In the Medical Department we want to strengthen the work with discharges across our wards. Contribute to the development and implementation of workflows for transitions between the hospital and the new health and care placements (SOP placements) in collaboration with relevant departments and municipal partners.

About the position

You will be organizationally attached to the Medical Ward but will have an cross-cutting role across the Medical Department’s wards. You will not be responsible for a fixed patient ward, but work with identifying patients where particular effort can make a difference for the discharge and the further course. As a coordinator you have a special responsibility to create overview, take initiative and ensure progress in discharge processes across the department. You will work closely with the wards’ staff and the flow function in AVA.

Your tasks will include

Support patient flow and discharge planning

  • Create daily overview of patients across the ward areas and identify opportunities for discharge.
  • Bounce ideas with the treating teams and the flow function in AVA about patient flow, capacity and solutions that can reduce unnecessary waiting.
  • Support early discharge planning based on the patient’s overall situation and needs.

Coordinate complex discharges

  • Manage the coordination of patient pathways with special or complex discharge needs in collaboration with the treating team. This can include patients with complex care and treatment needs or needs for several municipal interventions and partners.
  • Involve patients and relatives in planning the further course.
  • Coordinate with municipalities, home care, rehabilitation, health and care placements and other relevant partners.
  • Participate in cross-sector meetings and ensure clear agreements, follow-up and allocation of responsibilities.

Be a professional sparring partner

  • Advise and spar with colleagues in discharge planning, SAM:BO, documentation and cross-sector collaboration.
  • Contribute to teaching and knowledge sharing about SAM:BO, good discharges and sector transitions.

Contribute to development of practice

  • Identify challenges in discharge pathways and contribute to quality development through development and testing of workflows, data follow-up and audits.
  • Contribute to the development and implementation of workflows for transitions between the hospital and the new health and care placements (SOP placements) in collaboration with relevant departments and municipal partners.

Who are you?

We are looking for an experienced and professionally grounded nurse who is interested in coordinating patient pathways and collaboration across sectors.

We imagine that you

  • Have solid clinical experience, preferably from a ward.
  • Have experience with discharge planning and preferably complex patient pathways.
  • Are familiar with the collaboration between hospital and municipality, including SAM:BO.
  • Work systematically and can create overview of many simultaneous patient pathways.
  • Are good at collaborating, guiding and sparring with different professional groups.
  • Have an eye for both patient needs, clinical quality and the department’s overall operations.
  • Are interested in teaching, sharing knowledge and contributing to the development of workflows.

Experience with quality development and improvement work is an advantage, but the most important is your clinical understanding and ability to translate it into good solutions.

About the Medical Department

The Medical Department at Lillebælt Hospital conducts evaluation, treatment and care of patients within several medical specialties. We have a broad patient group with both acute and longer-term courses, often with multiple simultaneous diseases and complex needs.

We work purposefully with high professional quality, patient safety and coherent patient pathways. This requires close cooperation between the department’s professional groups and with our municipal and other external partners.

As a discharge coordinator you will play an important role in strengthening this collaboration and developing a common direction for discharge practice across the department.

The position is under development, giving you the opportunity to influence both workflows and the future effort. In time the function may possibly be expanded to include the department’s outpatient clinics.

What do we offer?

We offer you

  • An cross-cutting function with the opportunity to influence patient pathways and workflows throughout the Medical Department.
  • A varied workday with clinical coordination, professional sparring, teaching and development.
  • A close collaboration with the department’s nurses, doctors, other professional groups and municipal partners.
  • Opportunity to apply your clinical experience in new ways and influence the development of discharge practice.

The position is organizationally attached to the Medical Ward. Working hours follow the same weekend structure as other positions with a special function, i.e. minimum 25% on-call work.
