About the job

Ambitious nurse with a passion for clinical leadership and coordinating citizen care pathways is sought

Do you have high professional ambitions, and are you passionate about delivering high-quality nursing care? Are you motivated to take on clinical leadership and coordinate coherent citizen care pathways in close collaboration with home care? Then you may be the person we are looking for to join our nursing group in Vallensbæk Municipality.

We are a small, independent nursing group with strong professional commitment and a shared desire to create the best possible trajectories for the municipality's citizens. In Vallensbæk Municipality we work from an "outside-in" perspective, where the citizen's wishes, needs and life situation are always the natural focus of our effort.

As a visiting nurse you will become a central professional resource in the organization. Based on the Elderly Act, you will be attached to a fixed home care team, where you play an important role in the nursing-coordinated management of citizen care pathways in close dialogue with citizens, relatives and home care staff. The nursing group is self-referral-based, and you will therefore be responsible for assessing and initiating nursing interventions based on the Health Act, our service catalog and your own nursing competencies.

We are looking for a colleague who, together with us, wants to strengthen learning, development and quality across the organization. You see the value in sharing your knowledge and contributing so that both citizens and colleagues experience continuity, security and high professional quality. We work proactively with early detection in close collaboration with home care and meet daily to ensure a common direction and coordination around the citizens.

With us you get

You will become part of an organization where quality is a shared ambition, and where cross-disciplinary collaboration is a natural part of everyday life. The nursing group consists of both younger and experienced nurses who tackle a wide range of tasks including palliative care, complex wound care and medication management.

We place a high priority on professional development. The group is led by a nursing professional leader, and there are weekly monodisciplinary meetings with a focus on learning, sparring and quality development. Our nursing professional work is also supported by a quality consultant, documentation nurses and IT coordinators, all of whom have extensive knowledge of practice and are close to daily operations.

The implementation of the Elderly Act is well underway, and at the Center for Care and Wellbeing we work purposefully to create cohesive solutions for citizens. You will be an active part of the interdisciplinary collaboration with home care, prevention counselors, rehabilitation counselors, case managers, dementia coordinators and other partners. Together we ensure holistic care pathways focused on quality, continuity and the citizen's needs. We all work from our shared core task:

Through dignified relationships and high professional standards, together with the citizen we create the opportunity for each person to live the best life every day.

We offer a thorough and structured training program with individual introduction, so you have the best conditions to get off to a good start. In addition, you have a strong influence on scheduling your working hours through the wish plan in KMD Opus Shift Planning. Our MED committee actively works for a good working environment, high well-being and a strong collegial community, because we believe that job satisfaction and professional quality go hand in hand.

We imagine that you

Are an engaged and reflective nurse with a view to both dignified relationships and high professional quality. You are ambitious on behalf of the profession and are motivated to develop yourself, your colleagues and the organization around you.

You thrive in the role of clinical leader and naturally take responsibility for coordinating complex citizen care pathways. You actively contribute to the nursing professional development, among other things through guidance, sparring and teaching of colleagues in both nursing and home care. If you have experience with the improvement model, that is an advantage. Otherwise we would like you to be eager to learn it.

You work systematically and with a professionally sound approach, collect relevant information and create a basis for good clinical decisions for the benefit of the citizen. At the same time you maintain an overview in acute and complex situations and have an eye for the details that make a decisive difference.

You place documentation high and see it as an important prerequisite for quality, continuity and cohesion in the citizen care pathways. In addition, you take active responsibility for a good learning environment and contribute positively to both mono- and interdisciplinary collaboration.

Finally, you are responsible and a good colleague who contributes to a healthy working environment both professionally, physically and psychologically.
