About the job

Are you passionate about complex nursing, professional development and APN methods? Then you might be our new colleague in the nursing professional resource team!

We are looking for a nurse – preferably with experience in using APN methods – for an exciting position of 37 hours per week with the start date 1 January 2027.

Are you keen to work with complex citizen journeys, advanced and preventive clinical nursing and the development of nursing in practice? Are you motivated by making a difference for citizens with complex health problems while strengthening the professionalism of your colleagues?

Then we have an exciting opportunity for you!

We are in the process of establishing a new nursing professional resource team, and we are seeking a colleague who wants to help shape, develop and set the direction for the team’s tasks and professional practice.

A new team with great professional opportunities

You will become part of a team of four colleagues, where we together will develop our tasks, workflows and collaboration across Skanderborg Municipality.

We aim to create a strong professional platform that supports municipal nursing in practice, qualifies complex citizen journeys and contributes to citizens receiving the right nursing at the right time.

We work with a focus on high professional quality, patient safety and a preventive approach. Our ambition is to prevent unnecessary hospitalizations or deterioration in citizens’ health and strengthen the coherence of citizens’ journeys.

So you will not only get a place in a new team – you will also have the opportunity to bring your experiences and competencies into play and help shape how we solve tasks going forward.

About the job

Your day-to-day life will be characterized by professional challenges, independence and close interdisciplinary collaboration with municipal nurses, general practice, discharge team, emergency team, hospitals and other partners.

You will, among other things, work with

  • Citizen journeys with APN methods and clinical assessment – The resource team handles complex citizen journeys with health problems. We are tasked with preventing unnecessary hospital admissions and strengthening quality and coherence for citizens with complex illness trajectories. This can include citizens who are multi-ill, have many readmissions, polypharmacy and reduced health literacy, etc.

Using APN methods and clinical assessment you contribute to uncovering the citizen’s overall health situation, identifying needs and supporting relevant actions.

Through citizen involvement, cross-disciplinary and cross-sector collaboration you help create an overview, coherence and flow in the journey for the citizen, staff and possibly relatives.

Through evaluation of each journey we translate our joint professional knowledge and experience into concrete improvements in everyday life.

  • Professional sparring and competence development – It is the resource team’s task to contribute to learning and competence development for all professional groups working with nursing tasks. This is based on knowledge about practice and evidence-based knowledge. This can include: teaching, sparring, guidance, accompaniment, auditing, or other methods requested by practice.
  • Teaching in Cardio-Pulmonary Resuscitation – The resource team provides teaching 1 day a week to staff within health, care and disability in Skanderborg Municipality.
  • Hygiene - as a resource nurse you are employed as a key person for infection control and must contribute to preventing infection spread and supporting practice during outbreaks.
  • Night nursing - the resource team works closely with the night nursing service and is scheduled with weekend night shifts every 4th weekend, where we have 1 night nurse and 1 resource nurse.

Your profile

We would very much welcome a nurse with an APN qualification (Advanced Practice Nursing) or a nurse who has experience with and interest in working with APN methods.

The most important is that you want to work systematically and holistically with complex citizen journeys and use your clinical professional skills to create better solutions for citizens. Also an interest in teaching and communication to colleagues across professional boundaries.

We imagine that you

  • Are a licensed nurse with a sharp clinical eye and a strong theoretical foundation.
  • Are interested in multi-ill citizens with diverse problems.
  • Are interested in cross-sectoral and cross-disciplinary collaboration.
  • Coordinate and organize nursing interventions based on the wishes of the citizen and the relatives.
  • Thrive in both independent work and in collaboration with others regardless of professional background.
  • Enjoy being a "guest in the house" and moving between several teams, and naturally enter into new professional relationships.
  • Have a focus on prevention, early detection and coherent citizen journeys.
  • Are interested in translating theoretical knowledge into improved practice.
  • Can take a leadership role towards partners and delegate shared responsibility.
  • Possess strong communication skills and can think creatively in complex situations.
  • Want to teach, guide and support other employees’ professional development.
  • Are curious, development-oriented and motivated to help build a new team.
  • Have good collaboration skills and can build relationships across professional groups and organizational units.
  • Have a driver’s license.

A part of a unified nursing professional resource team
