Are you ready to make a difference for residents in Middelfart Municipality?

We are looking for a committed nurse or therapist for our Department of Authority and Visitation – in a permanent position with start on 1/11-26 . The position is 37 hours per week.

You will be part of a strong team that works to ensure that residents receive the right help at the right time.

About the job

As part of Authority and Visitation we work with

• Comprehensive visitation and case processing under the Elderly Act and the Social Services Act.
• Coordination of discharges from hospitals
• Assessment and allocation of help under Elderly Act §§10, 13, 16 & 17 and under SEL §§ 83, 84, 86 as well as the General Housing Act.
• Interdisciplinary collaboration with a focus on creating coherent citizen pathways.

We are working towards a more citizen-centered visitation , where we strengthen cooperation with home care and nursing.

We work from a rehabilitative approach and have a focus on meeting citizens based on their wishes, needs and dreams. You will join a department that is constantly developing to be a more citizen-centered and efficient partner.

About you

We are looking for someone who

• Is health-professionally educated – nurse or therapist
• Has experience as an authority officer or can see yourself in that role
• Can apply legislation in practice
• Thrives on making decisions and communicating them clearly
• Has a humble approach in contact with citizens – we want to be known for being professional, friendly, approachable and empathetic
• Has strong communication skills – both written and spoken
• Places the citizen first and thinks in holistic solutions
• Is structured, innovative and good at collaboration
• Holds a driver’s license and can plan your own workday

As a person you are approachable, relationship-building and contribute to a good working environment.

Salary and terms of employment

Salary and employment are in accordance with the applicable collective agreement.
