About the job

Become an SKP worker in the Homelessness Initiative

Do you want to work outreach-oriented, relationally and persistently with people who are homeless or at risk of losing their housing?

Based on the Government's 10-year plan for Psychiatry, the Homelessness Initiative in Horsens Municipality is expanding the SKP team with one position – we are therefore looking for an engaged SKP worker (support and contact person) with a start date of January 4, 2027 .

As an SKP worker you will be part of an initiative for some of the municipality's most socially vulnerable citizens. These are people who can be in complex life situations marked by, for example, homelessness, mental health vulnerability, substance-use issues, loneliness and limited or no contact with the established help systems.

Who are we?

The Homelessness Initiative is part of the Centre for Psychiatry and Socially Vulnerable in Horsens Municipality and has offices at Horsens Municipality’s Town Hall.

Our common starting point is the vision New Paths – New Lives . This means, among other things, that the citizen must be a guide in their own life. We base our approach on the citizen's wishes, hopes, resources and current situation and work to ensure that the individual can live as independent and meaningful a life as possible.

As an SKP worker you will be part of the overall homelessness effort, where the goal is to prevent and reduce homelessness and create coherent pathways for each citizen.

We believe that lasting solutions are created through collaboration. Therefore you must be curious about other professional disciplines and able to collaborate across organizational boundaries.

You must also be able to navigate independently in a workday that cannot always be planned in advance. We meet people who do not always seek out or can maintain contact with existing services. Therefore the job requires that we can be outreach-oriented, flexible and persistent – while at the same time respecting the citizen's right to determine their own pace and direction. We put the task in front of the person . This means that the work starts from the citizen's needs and goals and a shared professional approach.

We create security and trust in contact, while working systematically and professionally on the task we are brought into the world to solve.

Your task as an SKP worker

As an SKP worker you must be able to establish contact with people who may have difficulty using the existing social and health services.

A large part of the work takes place where the citizen is. It can be on the street, at drop-in centers, in temporary housing, in the citizen's home or other places in the local area.

You must be able to meet people respectfully and without predetermined requirements for how change should look.

Among your tasks will be

  • work outreach-oriented and create contact with people who are homeless or severely socially vulnerable
  • establish and maintain a trusting contact – even when the contact is sporadic or difficult
  • uncover the citizen's wishes, needs and possibilities together with the citizen
  • support the citizen in contact with relevant municipal, regional and voluntary services
  • build a bridge to, for example, housing initiatives, authorities, healthcare, psychiatry, substance-use treatment and civil society
  • support the citizen's path toward a more stable housing and life situation
  • work rehabilitative and with respect for the citizen's self-determination
  • collaborate closely with colleagues and relevant partners across professional areas

You must be able to accept that development does not necessarily occur linearly. Sometimes progress is that a citizen opens the door. Other times it is that the citizen accepts help with a concrete problem or resumes contact after a long absence.

It requires both patience, professionalism and the ability to hold on to hope and possibilities together with the citizen.

We are looking for you who

Has a relevant social or health profession education.

We also expect that you

  • have experience working with people who are homeless and socially vulnerable
  • thrive with outreach work and can work independently
  • are persistent and can maintain contact even when collaboration is challenging
  • can build trust without taking over responsibility for the citizen's life
  • have an eye for the citizen's resources, wishes and possibilities
  • can work flexibly and maintain an overview in complex situations
  • have good collaboration skills and see interdisciplinarity as a strength
  • are curious and reflective and actively contribute to professional development

It is advantageous if you have experience with the homelessness sector, psychiatry, substance-use sector, Housing First or other interventions aimed at socially vulnerable adults.

It is also advantageous if you have a health-professional perspective or experience.

We offer

A workplace where you have the opportunity to make a concrete difference for people who find themselves in vulnerable and complex life situations.

You will become part of a professional environment with engaged and experienced colleagues who show a high degree of independence and professional responsibility and where there is close cross-disciplinary and cross-sector collaboration.

There is a strong focus on professional reflection and development and a varied workday, where two days are rarely the same.

We are a dynamic workplace where well-being, professionalism and participation are highly valued.

There is a high degree of flexibility in how we solve tasks, and you will have the opportunity to influence the development of the Homelessness Initiative.

Our workplace community is built on trust in other people's abilities. We are open to different perspectives, and we see professional reflection, curiosity and the courage to try new paths as a prerequisite for development.

We value diversity and believe that different professional backgrounds, experiences and perspectives make us better at solving the task.
