Home Care Team Thorsvej is seeking a skilled social- and health care assistant who will help raise the professional level in our evening shift team and contribute to ensuring that our citizens experience a safe and coherent effort.

• We have a close collaboration with the nursing service and start the evening shift with a cross-disciplinary triage meeting, where good solutions are found together

Your main work tasks are

• Personal care and practical assistance to citizens in their own homes
• Delegated nursing services
• To observe, respond and relay when you notice changes in a citizen
• To contribute to citizens experiencing safety and continuity
• To use your professional skills and actively participate in the evening shift meetings with the nursing service

You are

• Trained social- and health care assistant – you do not need to have many years of experience
• A good colleague who is good at collaborating, and someone you can rely on
• Not afraid to ask for help when you need it
• Good at observing and acting when you notice changes in a citizen
• Curious and eager to develop yourself and your professional competence
• Good at creating calm – even when changes occur and you have a new citizen on your schedule
• Good at documenting in the citizen journal – we use Nexus

About the position

The position is 37 hours per week in the evening shift starting January 1, 2027.

We are flexible if you prefer a lower number of hours.

You must have a driving license, and we will provide a municipal car.

Terms of employment and deadline for application

The application deadline is December 15, 2026 at 8:00 AM.
