Professional coordinator - route planning for integrated home care

As part of a new organization of home care, we are bringing together our professional coordination; the route planning shall create greater coherence, strengthen collaboration and ensure consistent quality in task execution. Therefore we are seeking professional coordinators who will help set the direction for the future of home care and ensure strong, coherent citizen journeys across disciplines and sites.

Your most important task

You shall ensure that citizens receive the right help at the right time, and that interventions are planned and coordinated with high professional quality. At the same time you should support operations where resources are used as effectively as possible, and where continuity, quality and holistic care go hand in hand.

You will become a central link between citizens, staff, nursing, therapists, visitation, shift planning and management, and you will have a key role in the daily coordination of home care interventions.

Your tasks

• Coordinate and follow up on citizen journeys, returns home and discharges
• Ensure correct initiation of new citizens and coherent interventions
• Distribute daily tasks based on citizens' needs, competencies and continuity
• Develop and maintain template plans and planning basis
• Quality assure routes and hand over thoroughly worked-out routes to shift planning
• Support efficient use of resources and a high delivery rate
• Advise staff in professional priorities and use of Nexus
• Contribute to the development of workflows and a consistent planning practice
• Ensure quality in documentation, registrations and follow-up on deviations

We are looking for you, who

• Are a licensed nurse or social and health care assistant
• Have experience from home care, district nursing or similar area
• Have strong competencies within coordination, allocation and planning
• Have in-depth knowledge of Nexus and municipal home care operations
• Can create an overview and make qualified decisions in a busy everyday life
• Work structurally, systematically and independently
• Communicate clearly and respectfully
• Have a natural understanding of the connection between professional quality, operations and economy
• See the value in collaboration and shared solutions across the organization

Practical information

• The position is full-time, weekends off, starting December 1, 2026 (or by agreement)
• Application deadline: Sunday, November 15, 2026 at 23:59.
• First interview will be held on Tuesday, November 17, 2026 from 12-16
• Second interview will be held on Tuesday, November 24, 2026 from 12-16 and there will be a case from everyday work that the interview will be based on.
• There will be a personality test between the first and second interview
• Salary according to the applicable collective agreement and qualifications
