About the job

We are looking for a planner for the Sorø Town Home Care Group

Would you like to help ensure holistic care and coherent citizen journeys for our citizens?

Do you have strong coordination skills, a good overview, and thrive in a daily life where plans must quickly adapt to reality?

Then you may be our new planner in the Sorø Town Home Care Group.

We are seeking an engaged and structured planner with a health profession background, who wishes to play a central role in daily operations. Together we work to ensure holistic care for citizens through the right effort, at the right time and with the right competencies in relation to each individual citizen. As a planner you will be a key person in the collaboration between citizens, employees and management, where you contribute to quality, continuity and an efficient use of resources.

About us

Sorø Town Home Care consists of two care groups: one group in Sorø town and one group in the countryside around Sorø. Both groups are organized into two smaller teams to ensure proximity, continuity and strong relationships with the citizens.

You will take responsibility for planning day and evening shifts, which carry out visits to Sorø town’s citizens as well as for the staff who cycle between the citizens.

You will be part of the entire Sorø Municipality’s home care planner team and in daily work you will closely collaborate with the planner colleague who is responsible for the countryside group. Together you ensure cohesive operations across the groups, and you serve as each other’s sparring partners as well as cover for each other during holidays, days off and other absence.

With us you get

  • A meaningful daily life where your effort makes a real difference for citizens and colleagues
  • A thorough introduction and good onboarding to the tasks
  • A strong community with engaged colleagues who help each other and lift together
  • Opportunity to influence the development of our smaller teams and workflows
  • Close sparring and dialogue with management
  • Training in our systems if you do not already have experience with them
  • A workplace focused on professionalism, collaboration and development

Your primary tasks

  • Daily planning and coordination of citizen visits and staff resources
  • Ensure alignment between citizens’ needs and the competencies that are deployed
  • Collaborate closely with the group leader on managing citizen journeys and resources
  • Handle changes in operations and find good solutions when everyday life does not go as planned
  • Contribute to high quality and continuity in care

We are looking for you who

  • Are educated as Social and Health Assistant or nurse
  • Have experience with planning and coordination, preferably from home care or the elderly sector
  • Have good IT skills and preferably experience with Nexus, KMD Vagtplan and Targit
  • Have an interest in operations management and resource planning with focus on both citizens and staff
  • Work in a structured manner and can maintain an overview in a busy and ever-changing daily life
  • Are action-oriented and can make decisions when the situation requires it
  • Communicate clearly, respectfully and with recognition
  • Are collaborative and good at building relationships
  • Can work independently and take responsibility for your own tasks
  • Approach daily life with a positive, flexible and solution-oriented mindset

We believe that the best planning is created close to the citizens

Therefore we are seeking a planner who both understands the operations, the value of cross-disciplinary collaboration and the importance of creating cohesive journeys for citizens. With us you will have the opportunity to bring your professional and coordinating skills into play every day.

Application deadline is 19.11.26. Interviews are expected in week 48.

Sorø Municipality will obtain a criminal record check in connection with employment.

We look forward to hearing from you.

In Sorø Municipality we view diversity as a strength, and we encourage all interested to apply for the position regardless of age, gender, disability, race, religion and ethnic origin. Our values are: credibility, engagement and job satisfaction, dialogue, openness and recognition.

The municipality has just over 30,000 inhabitants and lies in central Zealand in a scenic area with lakes and forests. Sorø Municipality has about 2,500 employees – and we are also a smoke-free workplace.
