About the job

The position is as a fixed day shift of 37 hours or adjusted to your preferences for other hours.

From 2027 we are working on the project "Flexible working hours", so you will have the opportunity to choose to work either every other weekend or every third weekend.

• We are 6 small home care teams in Kalundborg, covering Røsnæs – Raklev – Saltbæk – Vollerup - Asnæs – Tømmerup - Melby and the town centre.
• We work in small teams, where you as a Social and Health Care Assistant (SSA) have great influence on your own tasks, in planning and agreements with citizens about help and support.
• You work with holistic care, the Health Act and holistic care under the Elderly Care Act.
• Close collaboration in the interdisciplinary team, which consists of colleagues in your own team, a case manager, occupational therapists and physiotherapists as well as nursing staff, around complex cases.
• You share responsibility for coordination and planning during the planner’s absence.
• You and your team work with continuity for the citizens, so to the greatest extent possible the same staff come to the homes in the individual shift.
• You work with early detection and intervention and ensure follow-up and monitoring of progress in the rehabilitative work.
• You have influence on shaping the collaboration in the groups.
• In your work you reflect our values: shared responsibility, dialogue, well-being, respect and pride.
• You participate in interdisciplinary collaboration where sparring is a natural part of everyday life. Recognition is one of the keywords in our dialogues.
• As a new employee, you will receive a thorough introduction in the group, in nursing and an onboarding training tailored to your needs.
• You will undergo an introductory course as well as a relocation/transition course.
