About the job

Do you want to follow the geriatric patient across sectors and pathways? Are you passionate about complex nursing, interdisciplinary collaboration and the possibility to make a difference for elderly patients? Then perhaps you are the one we are looking for!

We are looking for engaged nurses for a permanent position of 37 hours per week with start 1 January 2027. The position combines work in the Geriatric Outpatient Clinic – Center for Fall Assessment (CFU) and Geriatric Clinic (GK) – with work in the Geriatric Ward.

You will have a varied daily life, where you work with ambulatory assessment and follow-up and with care and treatment of hospitalized geriatric patients. You will become part of a strong multidisciplinary environment where professionalism, collaboration and cohesive patient pathways are in focus.

About half of your working time will be in the ward, where you will participate in shift and weekend work. The other part of the working time will be in the outpatient clinic.

You report to the Chief Nurse in the Geriatric Outpatient Clinic.

Two functions - one common task: The good patient pathway Geriatric Outpatient Clinic – assessment, treatment and follow-up

In the Geriatric Outpatient Clinic we work with assessment, examination, treatment and follow-up of elderly patients with geriatric problems. It takes place both in the hospital and, where relevant, in the patient's home.

We work cross-disciplinary and have a focus on creating cohesive pathways where the patient experiences safety and continuity. As far as possible, the patient has the same nurse as contact person throughout the course.

As a nurse you get a central role in coordinating the patient's course, creating overview and ensuring that examinations, treatment and follow-up hang together. You collaborate with the patient, relatives, doctors, municipal partners and other relevant professionals.

An important goal is to strengthen the patient's functional ability and quality of life and to prevent unnecessary hospital admissions.

Geriatric Ward – complex nursing and multidisciplinary treatment

In the ward we receive geriatric patients with complex problems that require a holistic approach. Here nursing is close to the patient, and you will have the opportunity to apply and develop your clinical competencies in an environment with high professionalism and close interdisciplinary sparring.

We have fixed associated physiotherapists, occupational therapists and a dietitian and collaborate closely with, among others, municipal home care and the patients' general practitioners.

Nursing staff have competencies within, among other things, dementia, wound care, nutrition therapy and incontinence. We work purposefully to ensure good patient pathways and create the best possible conditions for the patient's further rehabilitation and everyday life after hospitalization.

Your tasks and responsibilities

Across the outpatient clinic and the ward you will play an important role in ensuring high quality and coherence in the patient pathways.

  • Provide clinical nursing care based on the individual patient's needs, resources and wishes.
  • Carry out clinical assessments and make nursing professional decisions within your own area of competence.
  • Coordinate and follow up on patient pathways in collaboration with doctors and other professional groups.
  • Inform, guide and involve patients and relatives in care, treatment and follow-up.
  • Collaborate with municipal partners, home care, rehabilitation offers and general practitioners.
  • Document nursing care and contribute to quality and continuity in patient pathways.
  • Contribute actively to professional sparring, learning and development of nursing.
  • Help develop and improve workflows and patient pathways with focus on quality and evidence-based practice.

What do we offer you?

We offer you a position where you have the opportunity to apply your nursing expertise in several ways and follow the geriatric patient in different stages of a disease course.

  • A varied workday with a mix of outpatient nursing and care for inpatients.
  • A strong interdisciplinary community with engaged and competent colleagues who share knowledge and help each other.
  • A thorough introduction to both the outpatient clinic, the ward and the geriatric specialty.
  • Professional development and sparring with the opportunity to strengthen your competencies within complex geriatric problems.
  • Influence on patient pathways and the opportunity to contribute ideas for new solutions and improvements.
  • A good working environment where quality, collaboration and human values go hand in hand.
  • A high level of information and involvement, where we emphasise that employees' perspectives are heard.

We are a department with large professional engagement, high sense of responsibility and a desire to continually improve for the patients. We prioritize good communication, professional dialogue and collaboration where we jointly take responsibility for the task.

Who are you?

We are looking for nurses who want to work with elderly patients and are motivated to create good, cohesive patient pathways.

  • Have at least two years of geriatric experience from hospital or primary sector.
  • Have strong nursing competencies and thrive with complex patient pathways.
  • Work independently and have good skills in planning, coordination and prioritization.
  • Have good collaboration and communication skills and actively contribute to a positive professional community.
  • Meet patients and relatives with respect, presence and understanding of individual wishes and needs.
  • Thrive with a varied daily life and can see opportunities in working across outpatient and ward.
