Social and Health Care Assistant for day shift in Home Care Frederikshavn South

Therefore we are looking for an engaged and professionally grounded Social and Health Helper for day shifts who wishes to make a real difference for our citizens while also being part of a strong collegial community.

About the job

As an SSH on day shifts you will be a central part of the citizens' daily lives. You will meet many different people and situations, where with your professionalism and your engagement you contribute to the citizens experiencing safety, dignity and coherence in their everyday life.

You will among other things work with

• Personal care and support.
• Support for citizens' resources and independence.
• Observation and documentation of changes in citizens' condition.
• Collaboration with relatives and multidisciplinary partners.
• Contribute to ensuring high quality and continuity of care.

The position is 30 - 37 hours per week or as agreed, with a fixed weekend every other week.
