Social and Health Care Assistant for an Outreach Team in Birkebjerg Center, South-East District

With us: We are social- and health helpers, social- and health assistants, occupational therapist, nurses, physiotherapist, students, pupils, operation supporters and team leader. We work in day and evening shifts.

We are a team with a strong focus on professional sparring, knowledge sharing and development. We work purposefully to create the best possible balance between work and private life.

Over the last few years we have worked targeted with daily triaging, and the holding of these meetings is now handled by all professional groups. We plan our own routes and adjust our visits to the citizens' needs.

We hold citizen conferences with participation of visitator, so we ensure proximity visitation and close follow-up on the citizens' current and individual needs. We prioritise rehabilitation in cross-disciplinary collaboration, so that citizens become as self-reliant as possible.

We have individually tailored introduction programs, and you have the opportunity to put all your competences to use, both somatic and psychosocial.

We have a presence and clear leadership that aims to support and promote your professional development and ensure balance between work life, competences and resources.

About you: You are trained social- and health helper. You are open and approachable. You apply your professional knowledge in interaction with the citizen and relatives as well as in the collegial community. You believe that competence development is a natural part of the profession. You work professionally and focus on solutions and possibilities.

You want a close-knit job where you give a lot of yourself, but also get a lot in return, and see it as natural to collaborate across with the citizen, colleagues, relatives and other partners. You have a driving license.

You will be employed in mixed shifts and the shift plan can be prepared in collaboration with you.

Salary and employment terms are in accordance with the applicable collective agreement between KL and FOA.
