About the job

Sorø Municipality's nursing service seeks a new nursing colleague.

  • Are you passionate about interdisciplinary collaboration and have a rehabilitative mindset?
  • Can you see yourself as a nurse in the citizen's own home with a focus on the citizen's whole daily life/health issues?
  • Do you enjoy working in small teams and are you a cheerful and positive team player with a good sense of humor?

Then this position is for you!

The special team is seeking a nurse in a permanent position of 32 hours per week with varying shifts, day shifts (8-16), evening shifts (12-20) and 1 weekend shift every 6 weeks (time window 8-18). You can expect about 1 evening shift per week.

You must thrive in a daily life where plans change quickly, depending on discharges and urgent tasks at hand. Therefore it is also important that you have a broad professional foundation, as the tasks include parts of all specialties. You are, or become, a specialist in being a generalist.

The special team's primary target group is citizens who live in their own homes and receive home care and/or nursing services. We focus on reducing readmissions and creating security in discharge with preventive and coordinating efforts.

The special team also handles tasks in collaboration with the E-hospital, e.g., IV medications.

The special team's tasks are

  • To provide holistic nursing with a high degree of interprofessional collaboration.
  • To conduct thorough nursing assessments.
  • To identify subacute issues and initiate preventive actions.
  • To assess the potential for rehabilitative interventions and assess the citizen's functional ability in their own home.
  • Nursing tasks such as: medication dosing, medication administration, wound care, palliative interventions, CRP measurement, vital signs measurement (TOBS), INR testing, enteral feeding tube insertions, fall assessments, safety processes, urgent tasks from general practitioners.
  • Documentation and interprofessional coordination constitute a substantial part of the tasks.

About the special team

  • We are a team of 6 nurses and 1 physiotherapist.
  • We have close collaboration with visiting teams, the training unit, the visitation office, home care, the assistive devices team and general practitioners.
  • We stay updated on the rest of the municipality's health services, as we must be able to advise and refer citizens to our colleagues' expertise when we hand over the citizen.
  • We are physically located in the Health Centre and share premises with clinical nurse specialists, nursing management, and nursing operations planners.

We offer

  • Joint introduction of 7 days and thorough training in the special team
  • A daily life characterized by many tasks around citizens, though there is room to develop skills and focus on well-being, as we place a high priority on the working environment.
  • High degree of interdisciplinary collaboration. Selected visits are carried out together with a therapist.
  • A good atmosphere with engagement from management to staff and vice versa.
  • A workplace in a small municipality in central Zealand, where it is easy to reach by train, bus, and car.

About you

  • You are a trained nurse with a minimum of 3 years of clinical experience
  • You are broadly professionally well-founded and preferably with experience in geriatrics, cardiology, emergency department, gastroenterology or COPD (KOL)
  • Experience with NEXUS is desirable
  • You are good at communicating verbally and in writing with citizens and partners.
  • You value documentation highly to ensure continuity of care for the citizen in everyday life.
  • You are a team player, responsible, and have an interest in your own and colleagues' joint professional development.
  • You have a driver's license

Salary and employment terms

  • The position is 32 hours per week
  • Day and evening shifts as well as weekend work
  • Commencement on 1 December 2026
  • Salary and terms of employment are in accordance with the applicable collective agreement and the principles of local wage formation.
  • When employed we will obtain a criminal record check
